A reportable indident for hazardous materials (Reference 9 and 10) for the purpose of compliance with Reference 1 is defi,ed as one involving hazardous n

materials (poisons, explosives, compressed gases, or oxidizing, combustiole, flammable, irritating or corrosive materials) in which:

a.

A person is killed or hospitalized, or b.

Property damage exceeds $50,000, or C.

A situation exists of such a nature that there is concern that C.I.a or C.L.D. eay develop.

A reportable incident for radioactive materials is one in which there i s:

Any release exceeding technical specification limits, or b.

Any fire, breakage, spillage, or release from materials which are in transit away from nuclear station sites, or

'Any exposure or contamination of a person at c.

a level which exceeds 10CFR20 limits, or d.

A situation of such a nature that there is

Forward the completed Hazardous Material Inc i dent Report.(Attachment A) to the Administrative and Support Services Assistant Superintendent for review.

After the review, the form will be retained in the Station Central File.
