Re:St.,LucieUnits1and2DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort92-05VIOLATION(A):A.TechnicalSpecification4.0.5requiresinpartthatInserviceInspectionofASMECodeClass1,2,and3componentsandInserviceTestingASMECodeClass1,2,and3pumpsandvalvesshallbeperformedinaccordancewithSectionXIoftheASMEBoilerandPressureVesselCodeandapplicableAddendaasrequiredby10CFR50,Section50.55a(g),exceptwherespecificwrittenreliefhasbeengrantedbytheCommissionpursuantto10CFR50,Section50.55a(g)(6)(i).SectionXIIWV3411requiresthatvalvesbetestedonceeverythreemonths.Unit1IntakeCoolingWater(ICW)TemperatureControlValves(TCVs)14-4Aand14-4Bwereidentifiedinthelicensee'sISTprogramdatedJanuary3,1990.Contrarytotheabove,Unit1.ICWTCVs14-4A'and14-4BwerenottestedonceeverythreemonthssinceJanuary3,1990,andspecificwrittenreliefhadnotbeengrantedbytheCommission.RESPONSE(A):Thereasonfortheviolationwasatypographicalerroronthepartof-FPLwhichresultedinmiscommunicationofFPL'soriginalintenttoonlytestthesevalvesatcoldshutdown.2.Thesevalveswerebeingtestedduringcoldshutdownbecauseofconcernsthatcyclingthemcouldimposetransientsonotherportionsofthesystem.TheFPLsystemengineerhasdeterminedthatifproperprecautionsaretakenthesevalvescanbestrokedtothefailsafepositionquarterlyasrequiredbytheASMEcode.Unit1ICWTCVs14-4Aand14-4BwerestrokedonMarch12,1992,fortheirfirstquartertest.Additionally,theywerestrokedonMay6,1992,forsuccessfulcompletionoftheirsecondquartertest.Thepumpandvalveprogramhasbeenchangedtofailsafestrokethesevalvesquarterly.Additionally,theISTprogramhasbeenreviewedbytheISTengineerandsiteQAandnoothererrorswereidentified.3.AdministrativeprocedureAP1-0010125AandDataSheets8Aand8Barebeingchangedtohavethese,valvesstrokedtotheir"asfailed"positionquarterly.ThesechangeswillbecompletedbyAugust1,1992,tobereadyforthirdquartertesting.FullcompliancewasachievedonMarch12,1992.AdministratiyeprocedureswillbecompletedonAugust1,199 Re:St.LucieUnits1and2DocketNos.50-335and50-389ReplytoNoticeofViolationInsectionReort92-05VIOLATION(B):B.TechnicalSpecification4.8.1.1.2.e.4requiresinpartthatatleastonceper18monthsalossofoffsitepowershallbesimulatedandthatauto-connectedshutdownloadsareverifiedenergizedthroughtheloadsequencer.Contrarytotheabove,procedurestoaccomplishthesurveillancefailedtoadequatelyverifytheabilityofthe"C"IntakeCoolingWaterpumptoenergizefollowingasimulatedlossofoffsitepower.RESPONSE(B):1.DuetooversightduringthecreationoftheoriginalprocedureonOctober12,1984,onerelaywhichispartoftheLossofOffsitePower(LOOP)circuitswasnottested.'lectricalMaintenanceverifiedproperoperationofthelatchingrelaysfortheICWpumpswhichcausethepumpstorestartonthedieselgenerators.ThisensuredthatthefullLossofOffsitePowercircuitwasoperable.ThesetestswerecompletedonMarch3,1992,forUnit1andMarch2,1992,forUnit2.3.The'C'umpportionsoftheECCStestingproceduresforbothunitswererevisedtoadequatelytesttheLossofOffsitePowerfunctions.Thisincludedanupgradeof'C'ntakeCoolingWaterpumpandComponentCoolingWaterpumptestingmethods.4.FullcompliancewasachievedonMarch3,199
