Dear Resident:

This Department has the responsibility to prepare the plans for emergency response in your area for any incident or accident associated with the Palo Verde Nuclear Generating Station. We are requesting that you complete and return to us the enclosed postage-free information form to aid us in this effort. As you can see, this form will provide us with current information as to the number of people residing within 10 miles of the plant and alert us to the names and ad-dresses of those who may need transportation provided in the event of an emergency. Due to the design and type of construction used at the Palo Verde Nuclear Generating Station, we believe that when becomes operational it is extremely unlikely that an it accident will occur which would pose a threat to those living near the plant. However, Federal Regulations re-quire the preparation of Emergency Response Plans and that these plans be exercised on a regular basis. Thank you for your assistance. If you have any questions, please contact this office. Sincerely, Roy B. Bluhm Director RBB/br Enclosure Phoenix Scottsda le Glendale Participating Cities Mesa Tempe Chandler
