RENEWED FACILITY OPERATING LICENSE NPF-30 OWNERS ACTIVITY REPORTS (OAR-I FORMS) FOR CYCLE/REFUEL 22 Please lind enclosed the O\\ners Activity Reports (OARI Forms) Fot Cc1e 22 and Refuel 22 activities at Callaway Plant. The reports are submitted pursuant to the provisions of the applicable revisions of ASME Boiler and Pressure Vessel Code Case N-532. Alternative Requirements to Repair and Replacement Documentation Requirements and Inservice Summar Report Preparation and Submission.

The OAR-I forms enclosed are associated with the Following Cycle 22 and ReFuel 22 activities at Callawav Plant:

1. Inservice Inspection (1St) and Repait/Replacement activities for the first period of the Fourth ISI Program interval, and

2. Containment Inservice Inspection pursuant to ASME Section XL. subsection IWE For the third period of the second I WE Program interval.

Each enclosed OAR- I form includes a listing of items with identified flaws or relevant conditions that required evaluation for continued service (Table I) and an abstract of repair/replacement activities required for continued service (Table 2).

In accordance with Revision 0 of the fourth Interval ISI Program Plan, the OAR-i form for the ISI and Repair/Replacement activities is submitted in accordance with Code Case N-532-5. Similarly, in accordance with Revision 3 of the IWE Program Plan, the OAR-I form for the Containment Inservice inspection activities is submitted in accordance with Code Case N-532-4.
