Outage, Period and Interval Start and End Dates This Inservice Inspection Summary Report addresses examinations performed between the end of the previous refueling outage April 17, 2006 through the end of the most recent refueling outage. This most recent refueling outage was the 19th refueling outage for Quad Cities Nuclear Power Station Unit Two. The outage started March 3, 2008 and was completed March 30, 2008 when the unit returned to service.

This refueling outage was the only scheduled outage during the second inspection period of the fourth inspection interval, which began on March 10, 2003. The first inspection period of the fourth inspection interval ended on April 17, 2006. The second inspection period is scheduled to end March 9, 2010. This report covers examinations performed to the second period, fourth interval requirements.

Tests, examinations, repairs and/or replacements associated with American Society of Mechanical Engineers (ASME) Code,Section XI were witnessed or otherwise verified by the Authorized Nuclear Inservice Inspectors. Non-ASME Section XI or augmented examinations were not witnessed or otherwise verified by the Authorized Nuclear Inservice Inspectors and are marked "Not Required" or "N/A" on the applicable examination reports.

The Authorized Nuclear Inservice Inspectors are inspectors of the Hartford Steam Boiler Inspection & Insurance Company of Connecticut (HSB-CT). The HSB-CT Regional Office is located at 2443 Warrenville Road Suite 500, Lisle, Illinois, 60532-9871.

1.2.

Examination Program Requirements The examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Division 1, "Rules for Inservice Inspection of Nuclear Power Plant Components", 1995 Edition through 1996 Addenda were implemented for Class 1, 2 and 3 components except where written relief has been granted by the NRC. Written relief, in the form of Relief Request 14R-02 approving Risk Informed Inservice Inspection (RI-ISI) as an inspection program alternative for Code Categories B-F, B-J, C-F-1 and C-F-2, was utilized during Q2R19. The ultrasonic examination volumes for RI-ISI components were increased from the Section XI requirements to meet the RI-ISI Program.

The ultrasonic examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Appendix VIII, 1995 Edition with Addenda through 1996, as required by 10 CFR 50.55a, "Codes and Standards", and amended by the Federal Register Notice 64 FR 51370 dated September 22, 1999 is executed through the Performance Demonstration Initiative (PDI) Program according to the schedule defined in 10 CFR 50.55a(g)(6)(ii)(C),

for wrought austenitic piping welds, ferritic piping welds, bolts/studs, reactor vessel welds, nozzle to vessel welds, nozzle inside radius sections and full structural overlaid wrought austenitic piping welds.

Section XI Examinations Summary A combination of ultrasonic (UT), liquid penetrant (PT), magnetic particle (MT) and visual (VT) testing were performed prior to and during the 1 9th refueling outage on ISI Program components. Examinations were performed by General Electric (GE) and Exelon Nuclear personnel. Volumetric and surface examinations were compared with the recorded results of previous examinations for applicable Class 1, 2 and RI-ISI components. ISI inspections comprised of 59 volumetric (35 were RI-ISI, Category R-A welds), eight (8) surface examinations and 48 VT-1 and VT-3 examinations, and 12 component examinations by VT-2 were performed on Class 1, 2 and RI-ISI. components. Six (6) of the visual exams were scope expansion exams as a result of indications found on two hangers that required repair or adjustment on two hangers. The two hangers were returned to their design condition and received an acceptable VT-3 exam following adjustment. Two (2) visual examinations were follow-ups examinations in accordance with IWF-2220 (b). The subsequent examinations found no additional relevant conditions. Code cases used for ISI or Repair/Replacement activities are listed in the ISI Program Plan.

Table 3.1-1 A, "Quad Cities Unit 2 ASME Section XI Limited Coverage Summary" contains the volumetric examinations that did not meet coverage requirements of ASME Section XI Code (i.e. greater than 90 percent of the specified examination volume). These exams were limited by design specific geometry. Accessibility could not be improved.

Eight (8) of the nine (9) examinations were on nozzle welds that have received relief for limitations in the past. The 8 nozzle welds coverage increased over the previous interval examinations. The ninth limited examination was the volumetric exam of the reactor vessel top head to flange weld (RPV-THHF). Relief requests as required by 10 CFR 50.55a(g)5(iii) and 50.55a(g)5(iv) will be generated and submitted within twelve (12) months following the end of the inspection interval.

Pressure testing VT-2 examinations not related to repair/replacement activities are documented in Table 3.1-2, "Quad Cities Unit 2 Pressure Testing Summary". The Class 1 and 2 Vessel Leakage Test and the Reactor Vessel Head Flange Seal Leak Detection System Test were performed in accordance with the 1995 Edition 1996 Addenda of the ASME Code for the second period of the fourth Interval.

Containment inspections were completed for Unit 2 for the end of the 10-year interval in accordance with 1992 Edition through the 1992 Addenda of ASME Section XI Subsection IWE. All accessible surfaces were examined. No corrective actions were required. This constitutes the end of the 10-year interval for Subsection IWE only.

Exelon personnel performed functional tests on a total of fourteen (14) snubbers for operability per the Service Life Monitoring/Failure Trending Sample and ASME OM Code Subsection ISTD. Eight (8) of the fourteen (14) snubbers were ASME Section XI Code designated. Of the fourteen (14) snubbers tested, Three (3) were found unacceptable for continued operation. Twenty-one (21) of the thirty (30) expanded sample snubbers were ASME Section XI Code designated. Of the additional thirty (30) snubbers tested, five (5) snubbers were found unacceptable for continued operation. Five (5) unacceptable snubbers were replaced with Lisega hydraulic snubbers. The remaining three (3) unacceptable snubbers were replaced with like-for-like units. In addition, a planned replacement of five (5) snubbers with Lisega snubbers was performed as part of Service Life Monitoring. Three (3) of the snubbers removed for service life replacements were taken credit for as part of the snubber sample expansion since they met the sample expansion criteria specified by OM code ISTD 7.9.3. Snubber testing activities are summarized in Table 3.1-3, "Quad Cities Unit 2 Snubber Functional Tests". Seven of the eight snubbers that failed were ASME Section XI components (Table 3.1-3 lists Code and non-Code to encompass scope expansion).

Table 3.1-4 "Quad Cities Unit 2 Conditions Noted on ASME Code Components and Corrective Actions" summarizes the conditions noted during the examinations and corrective measures taken prior to returning the unit to service.

Table 3.3 identifies number and percentage of examinations completed in the fourth interval.

3.2. BWRVIP-75-A/Generic Letter 88-01 Augmented examinations required by BWRVIP-75-A, "BWR Vessel and Internals Project Technical Bases for Revisions to Generic Letter 88-01 Inspection Schedules", were implemented in this outage (see Table 3.2-1). Four Category C IGSCC Susceptible welds were examined including one dissimilar metal weld utilizing PDI qualified examination techniques. No relevant indications were found and examination coverage was greater than 90%. This is consistent;.with previous examination results.

3.3. Invessel Inspections (IVI)

The only ASME Section XI inspection credited during Q2R19 IVVI activities is the once-per-period B-N-I VT-3 inspection of the vessel interior. Credit is being taken for this examination due to nature and number of other IVVI examinations in the vessel annulus region per Relief Request 14R-15, "Alternative Requirements to ASME Section XI, B-N-I and B-N-2 using BWRVIP Guidelines, Fleet Relief," submitted April 19, 2007.

To implement the requirements of the Boiling Water Reactor Vessel Internals Program, (BWRVIP), GE was contracted to perform the In-Vessel Visual Inspections (IVVI). The following components and assemblies were examined:

a 158 welds and components on jet pump assemblies in accordance with the BWR Jet Pump Assembly Inspection and Flaw Evaluations Guidelines (BWRVIP-41 -R1),

a 12 core spray piping welds in accordance with the BWR Core Spray Internals Inspection and Flaw Evaluation Guidelines (BWRVIP-18-A),

Six core shroud vertical welds outside the beltline in accordance with the BWR Core Shroud Inspection and Flaw Inspection Guideline (BWRVIP-76),

292 steam dryer welds and components per GE recommendations and consistent with the re-inspection criteria in pending Revision 1 to BWR Vessel and Internals Project Steam Dryer Inspection and Flaw Evaluation Guidelines (BWRVIP-139).

No new indications and no change in previous indications that were left as-is were identified on the Unit 2 steam dryer, which required repair during Q2RI18.

The following augmented examinations were performed as part of the Q2R 19 IVVI activities:

All 20 jet pump main wedges were inspected for evidence of main wedge (WD-1) movement. Two jet pumps (#5 and #8) have main wedge conditions that Quad Cities is monitoring in particular. The condition of none of the jet pumps has changed since Q2R1 8 (and Q2R1 7). Quad Cities shall continue to monitor the condition of jet pump wedges during future IVVI visual inspections.

Inspected the eight Feedwater Sparger End Bracket surfaces for evidence of wear.

These inspections were a follow-up to evidence of wear from the pins observed during Q2R 18. Seven of the eight brackets showed evidence of wear similar to that observed during Q2R18. This is not a concern for the immediate future. The worst worn bracket still has greater than 90% of its original thickness remaining. Quad Cities will continue to monitor this condition while long-term repair options are being evaluated.

Follow-up inspections of previous indications to determine extent of condition were performed on the steam dryer wall support lugs. No additional damage was observed except for normal wear.

Follow-up inspections of previous indications on two steam separator gussets. No change in conditions was observed.

Visual Inspections Torus Bays 3, 6, and 16 As a result of inspections in accordance with ASME Section XI IWE 1992 Edition, 1992 Addenda VT-3 was performed under Work Order 1004253 for remaining portions of Unit 2 containment structures. Indications were identified in Bay 3 (IR 00748868), Bay 6 (IR 00748405), and Bay 16 (IR 00749065) that were acceptable per previous evaluation but require subsequent VT-i examination. The indications were mechanical damage prior to initial painting that had not been previously documented.

Hanger 1086-M-202 Loose nuts found on hanger anchor plate that had not been previously documented. IR 00743311 assigned WO 01111174 to correct (acceptance VT-3 Report NDE-094) and subsequent scope expansion. Expansion was complete with no additional issues.

Hanger 1010-M-203 Spring can out of tolerance was identified under IR 00743307 and-corrected under WO 01110676 (acceptance VT-3 Report VT3-081) and subsequent scope expansion. Expansion was complete with no additional issues.

Component ID Description / Resolution In Vessel Inspections Feedwater Sparger End Brackets Inspected the eight Feedwater Sparger End Bracket surfaces for evidence of wear.

Seven of the eight brackets showed evidence of wear similar to that observed during Q2R18. This is not a concern for the immediate future. The worst worn bracket still has greater than 90% of its original thickness remaining. Quad Cities will continue to monitor this condition. Indications to non-code areas were documented in IR's 00746384, 00746392, and 00748013 and accepted in EC 369931.

Steam Dryer Wall Support Lugs Follow-up inspections of previous indications to determine extent of condition were performed on the steam dryer wall support lugs. No additional damage was observed except for normal wear. Indications in non-code areas were documented in IR 00748544 and accepted in EC 369931.

Jet Pump #17 An auxiliary wedge was installed on the second of two restrainer bracket setscrews on Jet Pump # 17 to take up a 0.041" gap between the setscrew and the jet pump barrel. This was a proactive measure to minimize vibration of the jet pump assembly. Jet Pump #17 already had an auxiliary wedge on the other setscrew. This was documented in IR 00746799 and installed under EC 369869. The Jet Pump is a non-code component.
