Outage, Period and Interval Start and End Dates This Inservice Inspection Summary Report addresses examinations performed in the fourth inspection interval starting April 19, 2006, through the end of the most recent refueling outage for Unit 1. This most recent refuteling Outage was the 19' refueling outage for Quad Cities Nuclear Power Station Unit 1. The Outage started May 3, 2007 and was completed May 23, 2007 when the unit returned to service.

This refueling outage was the first outage during the second inspection period of the fourth inspection interval, which began on March 10, 2003. The first inspection period of the fourth inspection interval ended April 18, 2006. This report covers examinations performed to the second period, fourth interval requirements. One more outage remains in the Second Period for Unit 1, therefore examnination percentage requirements will not be required to be met for Unit 1 until after the refueling outage scheduled in May, 2009.

Tests, examinations, and repair/replacement activities associated with American Society of Mechanical Engineers (ASME) Code,Section XI were witnessed or otherwise verified by the Authorized Nutclear Inservice Inspectors. Non-ASME Section XI or augmented examinations were not witnessed or otherwise verified by the Authorized Nuclear Inservice Inspectors and are marked "Not Required" or "N/A" on the applicable examination reports.

The Authorized Nuclear Inservice Inspectors are inspectors of the Hartford Steam Boiler Inspection & Insurance Company of Connecticut (HS B-CT). The HSB-CT Regional Office is located at 2443 Warrenville Road Suite 500, Lisle, Illinois, 60532-987 1.

1.2.

Examination Program Requirements The examination requirements of ASME Boiler and Pressure Vessel Code, Section X1, Division 1, "Rules for Inservice Inspection of Nuclear Power Plant Components", 1995 Edition through 1996 Addenda were implemented for Class 1, 2, and 3 components except where written relief has been granted by the NRC. Written relief, in the form of Relief Request 14R-02, approving Risk Informed Inservice Inspection (RI-ISI) as an inspection program alternative for Code Categories B-F, B-J, C-F-I and C-F-2, was utilized during QIRJ9. The ultrasonic examination volumes for RI-ISI components were increased from the Section XI requirements to meet the RI-IS I Program.

The ultrasonic examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Appendix ViII, 1995 Edition with Addenda through 1996, as required by 10 CFR 50.55a, "Codes and Standards", and amended by the Federal Register Notice 64 FR 51370 dated September 22, 1999 is executed through the Performance Demonstration Initiative (PDI) Program according to the schedule defined in 10 CER 50.55a(g)(6)(ii)(C),

for wrought austenitic piping, welds, ferritic piping welds, bolts/studs, reactor vessel welds, nozzle to vessel welds, nozzle inside radius sections, and full structural overlaid wrought austenitic piping welds.

Section X1 Examinations Summary A combination of ultrasonic (UT), liquid penetrant (PT), magnetic particle (MT) and visual (VT) testing were performed prior to and during the I 9 th refueling ouitage on 151 Program components. Examinations were performed by General Electric (GE) and Exelon Nuclear personnel. Volumetric and surface examinations were compared with the recorded results of previous examinations for applicable Class 1, 2 and RI-ISI components. ISI inspections comprised of 46 volumetric (19 were RI-ISI, Category R-A welds), 10 surface examinations and 34 visual examinations were performed on Class 1, 2 and RI-ISI components. Five of the visual exams were subsequent exams due to IWE-2200(b) for corrective action from QLRI8. The subsequent examinations foulnd no additional relevant conditions. Code cases used for 151 or Repair/Replacement activities are listed in the IST Program Plan.

Table 3.1 -lIA, "Quad Cities Unit I ASME Section XI Limited Coverage Summary" contains the voluimetric examinations that did not meet coverage requirements of ASME Section XI Code (i.e. greater than 90 percent of the specified examination Volume). These exams were limited by design specific geometry. Accessibility could not be improved.

Eleven of the twelve limited exams during Q1RLR9 were on nozzle welds that have received relief for exam limitations in the past. The reactor vessel head to flange weld was also limited in coverage during its Q IR 19 examination and has also received relief in the past.

Relief requests as required by 10 CFR 50.55a(g)5(iii) and 50.55a(g)5(iv) will be generated and submitted within twelve (12) months following the end of the inspection interval.

Pressure testing VT-2 examninations not related to repair/replacement activities are documented in Table 3.1-2, "Quad Cities Unit 1 Pressure Testing Summary". The Class 1 Vessel Leakage Test and the Reactor Vessel Head Flange Seal Leak Detection System Test and Partial Class 2 system leakage tests were performed on outage accessible test blocks with the balance of the systems to be tested by the end of the period. The above tests were performed in accordance with the 1995 Edition 1996 Addenda of the ASME Code for the second period of the fourth Interval.

Containment inspections were completed for Unit 1 for the end of the 10-year interval in accordance with 1992 Edition through the 1992 Addenda of ASME Section XI Subsection IWE. All accessible surfaces were examined. No corrective actions were required. The completion of Unit 2 containment inspections scheduled in March, 2008 will constitute the end of the 10-year interval for Subsection I`WE only.

Exelon personnel performed functional tests on a total of eighteen (18) snubbers for operability per the Service Life Monitoring/Fai lure Trending Sample and ASME GM Code Subsection ISTD. Eleven (11) of the eighteen (18) snubbers were ASME Section XI Code

-designated. All eighteen (18) snubbers tested were found acceptable for continued

Four (4) of the eighteen (18) snubbers were tested for service life improvements. Of the four (4) snubbers tested for these service life replacemnents, one (1) PSA mechanical Snubber that was removed from the system was found acceptable, but exhibited signs of degradation. (IR# 629957). PSA mechanical snubbers have had a poor performance history on steam-affected systems due the effects of system vibration. Lisega hydraulic snubbers have demonstrated better reliability in these applications. This snubber was part of a planned replacement and upgrade to Lisega hydraulic snubbers. Snubber testing activities are summarized in Table 3.1-3, "Quad Cities Unit 1 Snubber Functional Tests".

Table 3.1-4 "Quad Cities Unit I Conditions Noted and Corrective Actions" summarizes the conditions noted during the examinations and corrective measures taken prior to returning the unit to service.

Table 3.3 identifies number and percentage of examinations completed in the fourth interval.

3.2. BWRVIP-75-AlGeneric Letter 88-01 Augmented examinations required by Generic Letter 88-01, "NRC Position on IntergranUlar Stress Corrosion Cracking (IGSCC) in BWIR Austentic Stainless Steel Piping" and BWvRVIP-75-A, "BWIR Vessel and Internals Project Technical Bases for Revisions to Generic Letter 88-0 1 Inspection Schedules", were implemented in the this outage. Two Category D IGSCC Susceptible welds were re-examined within 3 cycles of their previous examination utilizing normal water chemistry frequency requirements (Table 3.2-1). Implementation of PDI qualified examination techniques for stainless steel determined both welds 1OBD-F5 and 1OS-S3 to have no relevant indications. This is consistent with previous examination results.

3.3. NUREG-0619 In accordance with the "Alternate BWVR Feedwater Nozzle Inspection Requirements" for NUREG-0619, all four feedwater nozzle and inner radius volumes were examin~ed by shear wave automated technique using the GERIS system for Phased Array (No Threshold) during Q1R19. No rejectable indications were found. Automated examinations were supplemented by manual examinations to satisfy ASME Section XI requirements. Visual examinations of the flow holes and spargers located inside the reactor vessel were also performed by GE personnel at this time. This is in accordance with NRC SER TAC numbers M94090, MAS 189, and MAS 190.

To implement the requirements of the Boiling Water Reactor Vessel Internals Program, (BWRVIP), GE was contracted to perform the In-Vessel Visual Inspections (IVVI). The following components and assemblies were examined: Jet Pump Assemblies in accordance with the BWVR Jet Pump Assembly Inspection and Flaw Evaluations Guidelines (B WR VIP-4 1-A), Core Spray Internal Piping in accordance with the BWVR Core Spray Internals Inspection and Flaw Evaluation Guidelines (BWRVIP-18-A), Core Shroud Vertical welds in accordance with the BWR Core Shroud Inspection and Flaw Evaluation Guidelines (BWvRVIP-76), and the Steam Dryer in accordance with GE recommendations and guLidance provided in the Steam Dryer Inspection and Flaw Evaluation Guidelines (BWRVIP-139).

Follow-Lip inspections of previous indications to determine extent of conditions were performed on the following components: Reactor Pressuire Vessel attachments associated with the Feedwater Spargers, the Steam Dryer wall support lugs, the Separator Guide Rods, and one surveillance specimen holder lower bracket in accordance with the BWR Vessel ID Weld Inspection and Flaw Evaluation Guidelines (BWvRVIP-48-A). Previously identified indentations in the Reactor Pressure Vessel cladding were re-inspected.

A 10-year re-inspection of the core shroud repair hardware (tie rods) was performed during QR1R9. Additional detailed inspections were performed in accordance with GE and BWRVIP recommendations in response to findings at Plant Hatch in 2006. No recordable indications were identified.

During the IVVI examinations, nine (9) jet pumps were identified with evidence of main wedge (WD-1) movement. GE Indication Notification Reports (INRs) and Exelon Issue Reports (IRs) were generated to document these conditions (JR 628107 and IR 629471).

All of the wedge movement was determined to be minor and normal. Only Jet Pump #18 showed evidence of wear into the brackets, therefore additional inspections were performed on Jet Pump #18 as required by BWRVIP-41 1-A. The additional visual inspections were of the setscrews, restrainer bracket welds and the welds attaching the restrainer bracket to the Jet Pump riser. No other recordable conditions were identified. Quad Cities will continue to monitor the condition of jet pump wedges during future IVVI inspections.

All eight Feedwater Sparger End Bracket pins were inspected during Q1R19. This was a follow-Lip to minor bracket wear from the pins identified during Q2RZ 18. Three (3) of the pinheads were observed to be wearing into the bracket hardware during QIRI9 due to flow-ind~iced vibration (IR 628584, JR 628585 and JR 628801). This is not a concern for the immediate future. The worst worn bracket still has greater than 90% of its original thickness remaining. Quad Cities will continue to monitor this condition.

Previous indications in three Core Spray piping welds were measured visually and with UT. The flaw lengths were consistent with previous measurements. However, UT

Since there were no obvious physical discontinuities that could expla 'in the indications they were assumed to be cracks (IR 629736). The potential cracks were evaluated in accordance with BWR VIP A and the Core Spray Line and Core Spray Sparger Flaw Evaluation Handbook (GENE-0000-000O1-4070-01 -RL1). The potential cracks are structurally acceptable for at least two cycles. Leakage through the cracks is also bounded by the current Loss Of Coolant Accident (LOCA) analysis.I Previous indications on three Jet Pump diffuser welds were visually mneasured and were also found to be consistent with previous measurements.

Engineering evaluation ( 'EC # 0000355008.) was performed (as permitted by Relief Request 14R-07) onl the I A Recirculation Pump and determined the leakage to be acceptable.

__________________________In Vessel Inspections Core Spray Piping Welds Jet Pumnp WD-I Wedge Wear Feedwater Sparger End Brackets Steam Dryer Wall Support Lugs RPV Interior Cladding Indications approximately 1.6" and 1.4" long were identified via UT in Core Spray piping welds 2P?4a and 4P4a, respectively. Exelon Issue Report (IR) 629736 documents these indications. The indications were assumed to be cracks. The cracks were evaluated using guidance in BWRVIP-18-A and the Core Spray Flaw Handbook (GENE-0000-000I -4070-0l-RI). and are acceptable for at least two cycles of operation. Bounding leakages through the cracks were applied in Leakage Calculation GENE-0000-00l10-0362-LTI. Supplement I. and are bounded for at least 2 cycles by the assumptions in the Current Loss Of Coolant Accident ILOCA) analysis.

During the IVVI examinations nine (9) jet pumps were identified with evidence of main wvedge (WD-l1) movement. GE Indication Notification Reports (INRs.) and Exelon Issue Reports (IRs) were generated to document these conditions (IR 628107 and IR 62947 1). All of the wedge movement was determined to be minor and normal. Only Jet Pump #18 showed evidence of wear into the brackets, so additional inspections were performed on let Pump # 18 as required by BWRVIP-41 -A. The additional visual inspections were of the setscrews, restrainer bracket welds and the welds attaching the restrainer bracket to the Jet Pump riser.

No other recordable conditions were identified. Quad Cities will continue to monitor the condition Of jet pump wedges during future IVV I visual inspections.

On three of the eight Feedwater Sparger End Bracket pins, the heads of the pinls were observed to be wearing into the bracket hardware due to flow-induced vibration (IR 628584.

IR 628585 and IR 628801). These were discovered during follow-uip inspections in response to similar wear observed onl Unit 2 during Q2R 18 and throug~hout the industry. This is not a concern for the immediate future. The worst worn brackets still have greater than 90% of the original thickness remaining. Quad Cities will continue to monitor this condition. In the meantime, long-term repair options are being evaluated.

Vertical rub marks and small gouges were discovered on all four Steam Dryer Wall Support Lugs (WSLs) near the area where the brackets are welded to the pads. which in turn are welded to the RPV. These were documented in GE INR Q IRI19-IVVI-07-1 I and Exelon IR 628916. These gouges and rub marks are the result of dryer installation and removal. Some amount of material loss during dryer installation and removal is expected. The WSLs will be re-inspected during Q I R20 to confirm no new cracking initiates from any of the indications.

During follow-up inspections of two impact indentations in the RPV cladding at 295' two more indentations were discovered. These were documented in GE INR QI RI9-IVVI-07-15 and Exelon IR 629843. The new indications are attributed to mechanical deformation resulting from impact with the clad surface. They do not have IGSCC or fatigue characteristics. These dings and gouges are unlikely to propagate. The cladding is nonl-structural and its function is to minimize corrosion. The indications will be left as-is, but will be re-inspected during QIR2t). The prev ious indentat ions showed no d iscern ib le change.

Engineering Evaluation EC 365863 determined that all of the indications are acceptable for at least one cycle of continued operation.
