Relief Requested in Accordance with 10 CFR 50.55a(g)(5)(iii) for Volumetric Examination of Class 1 & 2 Components Ladies and Gentlemen: Pursuant to 10 CFR 50.55a(g)(5)(iii), Duke Energy Carolinas, LLC (Duke Energy) is requesting U.S. Nuclear Regulatory Commission (NRC) approval of a relief request to American Society of Mechanical Engineers (ASME) Section XI Code requirements for McGuire Nuclear Station, Unit 2 (MNS). Specifically, Duke Energy is requesting relief from paragraphs IWA-2200, IWB-2500, IWC-2500, and ASME Code Case N-716 for volumetric examination of certain Class 1 and 2 components on the basis that conformance with these ASME Section XI Code requirements is impractical as conformance would require extensive structural modifications to the component or surrounding structure. The relief request is provided in Enclosure 1 to this letter. Enclosure 2 provides non-destructive examination (NOE) data and coverage plots which detail the examination limitations. == Description:== Limited Volumetric Examination Coverage Component Number: See Table 1 for a List of Component IDs 2.0 APPLICABLE CODE EDITION AND ADDENDA: The McGuire Nuclear Station (MNS), Unit 2 Fourth Inservice Inspection Interval is based on the American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel (B&PV) Code, Section XI, 2007 Edition through the 2008 Addenda. MNS, Unit 2 maintains the responsibility to ensure exams were performed in accordance with the requirements of ASME, Section XI, Appendix VIII, Performance Demonstration for Ultrasonic Examination Systems, as amended and mandated by 10 CFR 50.55a. In the case of limited examinations, efforts were made to obtain additional examination coverage. Table 1 identifies if the examinations were performed in accordance with the requirements of ASME, Section XI, Appendix VIII. 3.0 APPLICABLE CODE REQUIREMENTS: 3.1. Examination Category B-D The extent of examination requirement for Examination Category B-D, Item Number B3.110, per Table IWB-2500-1, requires a volumetric examination of all nozzle-to-vessel welds as shown in Figure IWB-2500-7(b). 3.2. Examination Category C-A The extent of examination requirement for Examination Category C-A, Item Number C1.30, per Table IWC-2500-1, requires a volumetric examination of all tubesheet-to-shell welds, as shown in Figure IWC-2500-2. 3.3. Examination Category R-A The extent of examination requirement for Examination Category R-A, Item Number R1.11, per Table 1, Note 8 of ASME Code Case N-716, requires a volumetric examination of the piping base metal within 1/2 inch of the toe of the weld, and the fitting

Relief Request 13-MNS-002 was submitted and was approved by the NRC in order to utilize ASME Code Case N-716 for Periods 1 and 2 of the 4th Interval for Unit 2 (Reference 9.3). 3.4. ASME Section XI Examination Method IWA-2200(c) In accordance with ASME Section XI, IWA-2200(c), all nondestructive examinations of the required examination surface or volume shall be conducted to the maximum extent practical. When performing VT-1, surface, radiographic, or ultrasonic examination on a component with defined surface or volume, essentially 100% of the required surface OR volume shall be examined. Essentially 100% coverage is achieved when the applicable examination coverage is greater than 90%; however, in no case shall the examination be terminated when greater than 90% coverage is achieved, if additional coverage of the required examination surface or volume is practical. 4.0 IMPRACTICALITY OF COMPLIANCE: Pursuant to 10 CFR 50.55a(g)(5)(iii), relief is requested on the basis that conformance with these ASME Section XI Code requirements is impractical as conformance would require extensive structural modifications to the component or surrounding structure. 10 CFR 50.55a recognizes the limitations to Inservice Inspection of components in accordance with the ASME Section XI Code that are imposed due to early plants' design and construction. A design modification would be required to obtain ASME Code required coverage. Therefore, it is impractical for MNS Unit 2 to satisfy the ASME Section XI code requirements to perform a volumetric examination of the components listed in Table 1 due to the physical component configuration and interference from permanent plant equipment. Table 1 provides a summary of the examination limitations for each component for which relief is requested. The tables also indicate the outage the component was examined, the coverage percentage obtained for each component, and other pertinent design information. These tables are the cumulative lists of the limited ASME Section XI examinations performed during the Fourth ISI interval. Enclosure 2 provides non-destructive examination (NDE) data and coverage plots that detail the examination limitations. Accordingly, pursuant to 10 CFR 50.55a(g)(5)(iii), Duke Energy has determined that conformance with the code requirement of essentially 100% coverage of weld volume or area examined was impractical due to various constraints and limitations as stated above. Duke Energy requests NRC approval of the proposed alternative as stated below. 5.0 BURDEN CAUSED BY COMPLIANCE: Compliance with the applicable ASME Section XI Code volumetric examination requirements can only be accomplished by redesigning and refabricating the subject

Based on this, the ASME Section XI Code requirements are deemed impractical in accordance with 10 CFR 50.55a(g)(5)(iii). 6.0 PROPOSED ALTERNATIVE AND BASIS FOR USE: On the basis that obtaining the required examination coverage is impractical due to physical obstructions and limitations imposed by original plant design and fabrication including socket weld configurations, no alternative examination for additional ultrasonic testing (UT) examination coverage is proposed. As a minimum, all components completed the required examination(s) to the extent practical with regard to the limited component configuration or lack of access available. The examinations conducted confirmed satisfactory results resulting in no unacceptable flaws present, even though "essentially 100% coverage was not attained. Duke Energy has concluded that completed UT examinations in conjunction with VT-2 system pressure tests are an acceptable alternative that provides reasonable assurance of continued structural integrity Radiography (RT) is not a desired option because it is limited in its ability to detect service-induced flaws. Use of other manual or automated UT techniques, whether conventional or phased array, were considered, but would not increase coverage due to limitation created by the component configuration. 6.1. Examination Category B-D MNS Unit 2 has performed the ASME Section XI Code required examination to the maximum extent practical (Code Coverage), which is documented in Table 1. Due to the design of the pressurizer which includes heater wells which limit the available scanning area and nozzle geometry, consistent with previous exams, this examination incurred limited coverage. There are currently no alternative examination techniques available to increase coverage. Phased array UT examination was considered but due to the size of the qualified transducers, it proved prohibitive to exam coverage. In addition, VT-2 visual examinations associated with the Class 1 system leakage test are performed each refueling outage. The volume of coverage obtained during the UT examinations along with ongoing leakage tests every refueling outage provide adequate assurance that any flaw(s) that might have propagated through the subject welds are identified and dispositioned through the corrective action program. Thus, Duke Energy believes the completed exams provide an acceptable level of quality and safety. 6.2. Examination Category C-A MNS Unit 2 has performed the ASME Section XI Code required examination to the maximum extent practical (Code Coverage), which is documented in Table 1. The examination was limited due to the location of permanent support structure adjacent to the weld, consistent with previous exams. There are no alternative examination techniques currently available to increase coverage. In addition, VT-2 visual examinations associated with the Class 2 system leakage test are performed periodically.
McGuire Nuclear Station, Unit 2 Relief Request # RA-24-0290 Page 4 of 11 The volume of coverage obtained during the UT examination along with ongoing Class 2 system leakage tests periodically, provide adequate assurance that any flaw(s) that might have propagated through the subject welds are identified and repaired prior to returning the plant to power operation. Thus, an acceptable level of quality and safety will have been achieved and public health and safety will not be endangered by allowing the proposed alternative examinations in lieu of the Code requirement. 6.3. Examination Category R-A MNS Unit 2 has performed the ASME Section XI Code required preservice examinations to the maximum extent practical (Code Coverage), which are documented in Table 1. The socket weld configurations caused coverage limitations with no alternative UT examination technique currently available to increase coverage. The close proximity of socket weld 2NC2FW40-15 with an adjacent valve and coupling, resulted in a limited coverage. Additionally, due to the close proximity of two socket welds: 2NC2FW40-13 and 2NC2FW40-14, a volumetric preservice examination of 2NC2FW40-13, meeting the requirements of Note 8 of ASME Code Case N-716, could not be performed. The new weld configuration was designed due to the location of permanent adjacent rupture restraints resulting in a close proximity of welds 2NC2FW40-13 and 2NC2FW40-14. The affected piping was redesigned in accordance with an engineering change initiated from identified piping flaws, including a through-wall flaw, caused by back leakage through one or more of the cold leg injection check valve assemblies resulting in thermal fatigue. The check valves were subsequently redesigned and replaced with larger 2" check valves which required a reconfiguration of piping and associated welds. The ASME Section III Construction Code surface (Dye Penetrant - PT) examinations were performed and revealed no relevant conditions with 100% coverage obtained. In addition, VT-2 visual examinations associated with the Class 1 system leakage test were performed. The volume of coverage obtained during the UT examinations along with ongoing system leakage tests every refueling outage provide adequate assurance that any flaw(s) that might have propagated through the subject welds are identified and repaired prior to returning the plant to power operation. 7.0 DURATION OF PROPOSED ALTERNATIVE: This request is proposed for the MNS Unit 2, 4th Inservice Inspection Intervals identified below: MNS Unit 2, Fourth Inservice Inspection Interval: Start Date: July 15, 2014 End Date: February 29, 2024

- 2. ASME Code Case N-716, Table 1, Note 8, requires a volumetric examination of the piping base metal within 1/2 inch of the toe of the weld for socket welds categorized as R1.11 (thermal fatigue). During UT examination of socket weld 2NC2FW40-14 in the fabrication shop, the base metal exam volume covered a portion of the required exam volume for the adjacent field socket weld 2NC2FW40-13 (47.5% aggregate coverage achieved for base metal associated with 2NC2FW40-13 prior to welding). However, the required base metal exam volume after socket weld 2NC2FW40-13 was completed in the field could not be examined due to the close proximity to socket weld 2NC2FW40-14. The associated VT-2 visual examination of the socket weld and fitting was performed during the Class 1 system leakage test revealed no relevant conditions.
