Timeline

  • Initial plans were for Lake Charles facility to manufacture structural and mechanical, ASME type, modules.
  • Safety related fabrication started in early 2010.
  • ASME certification was projected for 2010.
  • Vogtle was anticipating the first CA20 sub-module in July 2010 and V.C. Summer (VCS) by November 2010.
  • The combined license (COL) for both VCS and Vogtle issued in early 2012.

Timeline

  • Vogtle Unit 3 CA20 was set in March 2014 and May 2014 for VCS Unit 2.
  • Quality issues discovered during NRC inspections have been documented in publicly available inspection reports.
  • Nonconformances were resolved in accordance with applicable codes and standards, and in compliance with NRC regulations prior to fuel load.

Safety Culture Issues Consequences

  • Impact of Safety Culture Issues at Lake Charles Module Fabrication Facility on the AP1000 Construction Sites:

- Schedule delays

- Increased NRC oversight (both at Lake Charles and at the sites)

- CB&I Confirmatory Order EA-13-196 required additional training of site personnel

- Observed impact on moral of CB&I personnel at the site (increased allegation intake from the sites) 11

Safety Culture Issues Consequences

  • Impact of Safety Culture Issues (cont.):

- Licensees created additional quality control inspection teams.

- Licensees needed to adjust their original module fabrication plans:

  • New manufacturing facilities were needed to complete the module fabrication (for both ASME mechanical and structural modules).
  • Numerous fabrication nonconformances required rework at the sites.
  • Some modules were intentionally shipped to the sites partially complete.

- CA20 module was set > 2 years after the COL was issued, some delays could be attributed to safety culture issues.
