INITIAL NRC FORM 241 FOR CALENDAR YEAR 2022
Dear Mr. Thomson:
This acknowledges receipt of your initial NRC Form 241, Report of Proposed Activities in Non-Agreement States, Areas of Exclusive Federal Jurisdiction, or Offshore Waters, dated March 17, 2022, and the $2,700.00 application fee, submitted to the NRC Region I office to request authority to conduct licensed activities in NRC jurisdiction under the authority of a general license pursuant to Title 10 of the Code of Federal Regulations (10 CFR) Part 150.20.
Your Agreement State license is recognized as valid for the proposed use of licensed materials in NRC jurisdiction. Your general license under 10 CFR Part 150.20, shall expire on December 31, 2022, and must be renewed at that time if you want to continue the proposed activities after that date.
Working under reciprocity in NRC jurisdiction requires you to comply with the NRCs regulations, as described in 10 CFR Part 150.20. This general license authorizes you to conduct the same activity, as authorized on your State of New Hampshire license, in areas of exclusive Federal jurisdiction, non-Agreement States, or in offshore waters. Under the general license, reciprocity activities, including storage, are limited to a total of 180 days in a calendar year. The NRC tracks reciprocity usage on the basis of approved usage days and will not approve any activity, under the general license, that causes the total usage days to exceed 180 days, except for offshore waters. You should track the days of use and storage to ensure that the 180-day limit is not exceeded. If work activities are expected to proceed beyond 180 days in the calendar year, you must obtain an NRC license.
The NRC may perform inspections of activities by Agreement State licensees at the listed work site location(s) or at the home office address. Failure to comply with NRC regulations while conducting your activities in exclusive Federal jurisdiction, non-Agreement States, or offshore waters may result in NRC enforcement action(s). Enforcement actions against licensees could include issuance of a notice of violation; imposition of a civil penalty; or issuance of an order suspending, modifying, or revoking the general license as specified in the NRCs Enforcement Policy. The NRC maintains a Web Based Licensing (WBL) system for proposed Agreement State licensee activities conducted in accordance with 10 CFR Part 150.20. The calendar year 2022 Location Reference Number (LRN) (NRC Form 241, Block No. 17) for your proposed locations of work (NRC Form 241, Block No. 10) are shown in the following table:

Using the revised NRC Form 241 dated 01-2021, available at:
https://www.nrc.gov/reading-rm/doc-collections/forms/nrc241info.html, submit to this office when you propose to add locations of work, add or delete dates of work, change work site contacts, use different radioactive materials, or elect to perform additional work activities within NRC jurisdiction. Please note, if you choose to manipulate and save changes to the NRC Form 241 directly from the NRC Website, you will need the full Adobe Acrobat Reader software. All changes to the information submitted on the NRC Form 241 must include, as a minimum, the following information: (1) client name; (2) client telephone number; (3) complete or descriptive address of work location, including GPS coordinates if known; (4) work location telephone number; (5) dates of work; (6) list of radioactive material which will be possessed, used, installed, serviced or tested; (7) Agreement State license number; (8) expiration date of Agreement State license; (9) name of your contact at the work location; and (10) signature of the Radiation Safety Officer (RSO) or management representative.
The regulations require you to notify this office three days prior to beginning generally-licensed activity in the calendar year. A range of proposed dates not to exceed 180 days may be provided, when accompanied by a list of scheduled work dates and locations for that period. A change can be submitted by: (1) e-mail a PDF version of the NRC Form 241 to:
r1reciprocity.resource@nrc.gov (preferred) or (2) fax the NRC Form 241 using the facsimile number (610) 337-5269, to Attention: Jason vonEhr, Health Physicist, Division of Nuclear Materials Safety.
Enclosed is an approved copy of your initial NRC Form 241. This document and any future NRC Form 241 changes will not be made publicly available through the Agency-Wide Documents Access and Management System (ADAMS), the NRCs electronic document system.
