This letter transmits the report of the U.S. Nuclear Regulatory Commission (NRC) Operational
, % Safety Team Inspection (OSTI), conducted from March 15-31,1999, of activities authorized by

\\ NRC operating license DPR-65, at your Millstone Unit 2 facility. On April 7,1999, the inspection
N findir)gs were discussed with you and members of the Millstone Unit 2 staff at an inspection exit
meeting that was open for observation by members of the public.
._The OSTl performed an independent, broad scope assessment of management controls,
administrative programs, equipment, personnel and their readiness to support restart and safe
,
operation of the Millstone Unit 2 facility. The OSTI focused on the areas of management
programs and independent oversight; operations; maintenance and surveillance; and
engineering and technical support. Within these areas, the inspection consisted of selective
examinations of documents, direct observation of plant equipment and activities, and interviews

with plant personnel.
The team noted that significant progress had been made in enhancing overall station
performance. In particular, the team observed effective management oversight of plant
activities; an intrusive and independent quality assurance organization; and an improved
corrective action program. The operational evolutions observed during this inspection were all
conducted in a safe and controlled manner. The maintenance and engineering departments
provided timely and effective support in resolving emergent plant issues. The OSTI did not
identify any issues,'in addition to those already on your restart item list, that require your
attention prior to plant restart.
Based on the observations documented in the attached inspection report, the team has

concluded that, pending the completion of the restart issues identified in your tracking system,

the plant equipment, staff, management, programs and processes are in place to support a safe
restart and continued operation of Millstone Unit 2.

The NRC has also determined that one violation of NRC requirements occurred. This violation
is being treated as Non-Cited Violation (NCV), consistent with Appendix C of the NRC
Enforcement Policy. This NCV is described in the enclosed inspection report. If you contest the
violation or severity level of the NCV, you should provide a response within 30 days of this
inspection report, with the basis for your denial, to the Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, DC 20555-0001, with copies to the Regional
Administrator, Region I, and the Director, Office of Enforcement, United States Nuclear
Regulatory Commission, Washington, DC 20555-0001.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its
enclosures will be placed in the NRC Public Document Room (PDR).
