HIGHLIGHTS OF WEEKLY INFORMATION REPORT Week Ending July 30, 1982 Three Mile Island, Unit 1 GPU Nuclear has completed preliminary qualification testing of the explosive expansion repair technique to be used to repair steam generator tubes with defects within the upper tubesheet (UTS) area. A full-scale test is scheduled for August 5, 1982. The earliest estimated date to commence expansions in the TMI-l OTSGs is August 27, 1982.
Millstone Unit 2 A reactor shutdown was initiated on July 22 when two small reactor coolant leaks were identified. Startup is scheduled for July 27, 1982.
CRER On July 19, 1982, the CRER Progran Office issued a Draf t Supplement to the CRBR Final Environmental Statement.
Emergency Preparedness The Operations Center was activated on July 28, 1982 to participate in the emergency preparedness exercise at the Duane Arnold facility. NRC Head-quarters and Regional staff and FEMA representatives participated in the exercise.
OJality Assurance IE met with INFO representatives at the Hope Creek facility July 29-30, 1982 to observe a trial assessment of design and construction activities by INPO.
High Tenerature Gas-Cooled Reactor (HTGR)
RES was visited by the Gas-Cooled Reactor Associates (GCRA) General Manager and by DOE personnel July 22 to update NRC on the industry strategy plan and schedule for licensing a high temperature gas-cooled reactor (HTGR). GCRA intends to provide RES, by September, a copy of their licensing plan for the HTGR plant. The strategy plan indicates the identification of lead project candidate utilities in FY E2 (already accomplished) and site selection in FY E3.
International Programs DOE will host a major IAEA international conference on radioactive waste management in Seattle on May 16-20, 1983.

Ine Division of Contracts (DC) has awarded several cost-plus award fee contracts i
which contain incentives for contractors to perform more effectively and control costs. Under these incentive arrangements, Performance Evaluation Boarcs (PEBs) make periodic assessments of contractor performance to determine the amount of award fee warranted. The status of two (2) such assessments is outlined below:
(a) The PES for Contract No. NRC-03-81-130 with The Franklin Institute recently concluded its evaluation of Franklin's performance for the period October 1,1981 through March 31, 1982 and has forwarded its Award Fee Recommendation Report to the Fee Determination Official (FD0).
(b) The PEE for Contract he. NRC-03-82-096 with Science Applications, Inc.
(SAI) has concluded its evaluation of SAI's performance for the period March 1, 1982 through May 31, 1922 and has formulated its award fee recommendation. After apprising the contractor of the PEB's findings and recommendation, the PEB Chairman will forward the Award Fee Recommendation Report to the FD0 for approval.

I WEEKLY INFORMATION REPORT DIVISION OF FACILITIES AND OPERATIONS SUPPORT The last of the occupants of the Lugenbeel Building were moved to the Maryland National Bank Building on the weekend of July 31,1982 which completes the vacating of this building by NRC as a result of lease expiration.
Vacating this building involved moving all the Office of Resource Manage-ment, the Property Management Section, and three computer installations including the Payroll System, the Cashier's Office, and the Travel Services Branch, FOS, to offices in the Maryland National Bank Building. The total effort extended over more than one year and involved relocations of approxi-mately 3*0 work stations.

Three Mile Island, Unit 1 GPU Nuclear has completed preliminary qualification testing of the explo-sive expansion repair technique to be used to repair steam generator tubes with defects within the upper tubesheet (UTS) area. Following recommenda-tions by E&W and Foster Wheeler, GFU Nuclear has decided to expand the top 17 inches of all tubes within the 24-inch UTS and establish a six-inch sealed area free of defects which will be the load carrying seal. By this methed, all tubes with defects within the top 11 inches of the UTS can be saved. Consideration is being given to saving some additional tubes with defects belcw 11 inches by using a 23-inch expansion on selected tubes.
It has also been determined that each expansion will require two detona-
- iens; cne to expand the tube and,the other to tightly seal the tube against the UTS.

. D. C. ' Cook Unit 1 - Steam Generator Insoection During the present refueling cutage, a Westinghouse team inspected all four steam generators on July 17-24, 1982. Two loose cap screws and four nuts which were missing frcm the tube lane blocking device in steam generator
!2 were recovered in the sludge. No damage was revealed by the inspection.
Acoustic monitoring during operation did not detect loose parts. Failure of the tack welds connecting the nuts to a plate caused nuts and cap screws to drop from the blocking device. Full welds were made on all nuts in steam generators 2 & 4 to replace tack welds and. prevent failure. Inspet -
tion of steam generators 1 & 3 revealed that fuil welds were made on the nuts in tnese units during manufacture. The weld connection of nuts to hold steam generator tube lane blocking devices is believed to be peculiar to D. C. Cook Units 1 & 2.
A detailed report is being prepared by Westing-bcuse and a written report will be submitted to NRC by the licensee. The missing cap screws and r.uts in steam generator #2 were discovered because the biccking device is removed during each refuelir.g outage to provide access for sludge lancing.

Effluent and Environrental (Radiolacical) Information Liquid effluents from the TMI site released to the Susquehanna River after processing, were made within the regulatory limits and in accordance with NRC requirements and City of.Lantgster Agreement dated February 27, 1950.
During ne period July 23, 19E2, through July 29, 1952, the effluents contained no detectable radioactivity at the discharge point although indivicual effluent sources which originated within Unit 2 contained small amounts of radioactivity. Calculations indicate that less than three hundred-thousandths (0.00003) of a curie of tritium were discharged.

Reactor Euilding Entries. One reactor building entry was made during the last week in July. Tasks performed during the entry included the following:
Eeacter coolant system water samples were taken through the center control rod crive leadscrew orifice. Two samples were taken at the i
bottom of the plenum and one sample was taken from the area immediately above the core rubble bed.
Scaffoldine was brought into the reactor building in preparation for work to open a path through the 347 f t. floor for the personnel pcwer lift. The power lift is attached to the polar crane and is presently used to transpcrt workers from the 347 ft. elevation to the ;olar crare.
It is designed to eventually operate to the i
3D5 ft. elevation.
Flushing of vertical well surfaces below the 305 ft. elevation ccntinued. The flushing technique consists of a, rotating high pressure r.c::le which is lowered incrementally from the 305 ft.
elevation to the 2E2 ft. elevation. Approximately 7,000 gallons of processed water base been used for the flushes.

4 Ccre Insoection. The ccre inspection is sEheduled to continue on hecnescay, August 4, 1982. On Wednesday, workers will commente preparations to remove two additional control rod drive leadscrews to cor.tinue with the closed circuit television inspection of the core. A primary inspection objective will be an attempt to determine the lateral extent of the core damace. One peripheral leadscrew and one leadscrew between the periphery and the center will be removed to determine the lateral extent cf the void which was found in the center of the core.
The core inspection techniques will be similar to those used during the initial inspection. A pointed metal surface probe is being prepared and if ready, the prcoe will be used during the inspection next week in an atter; to determine physical characteristics of the rubble surface, e.g., is it compcsed of loose or congealed material.

The initial gas sample indicated a liner pressure of 0.1 psig with 6.3% bydrogen, 93.7% nitrogen and non-detectable oxygen. Subsequent sarples incicated hydrogen as high as 9.9%.
The liner was inerted with i
nitr: gen (as a cover gas) and the hydrocen concentration reduced to 0.EEi. Eecause hydrogen gas is ger.erated by radiolysis within the PF liners, the FF-3 gas composition will be monitored over a 14 day period to ceronstrate that conflammable conditions will exist over a minimum of twice the expected shipment period. Purificatien Demineralizer Disposition. As highlighted in the June 7, 1952 neexiy Status Report, preparations are continuing by GPU and DOE fcr the removal and disposal of two reacter coolant system

purification demineralizers. These 4 ft. diameter, 7 ft. high stainless steel vesseis contain up to 50,000 curies of mixed fission products deposited en icn-exchange resins. The initial activities will be to cetermine conditions within the auxiliary builcing demineralizer cubicles where dose rates are expected to exceed 1000 R/hr. An anendment was issued on July 23, 1982 to the Long Island Lighting Company (LILCO) to authorize the temporary storage of new fuel onsite at Shoreham Unit I, on the celivery trucks, while awaiting unloading and movement into the reactor building for storage. LILCO's entire possession limit for fuel assemblies arrived on 19 truckloads at the Shoreham facility at approximately 5:00 a.m. on Sunday morning, July 25, 1982.

i EN-82-25, Blanchard Valley Hospital (Findlay, Ohio) - A Notice of Violation and a Notice of Proposed Imposition of Civil Penalties in the amount of $1000 were issued on July 26, 1982. The action is based on alleged violations relating to an exposure of an individual while manually closing the shutter of a malfunctioning cobalt-60 teletherapy unit and the licensee's failure to report the occurrence to the NRC within twenty-four hours.

CFFICE CF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending July 30, 1982 High Temperature Gas-Cooled Reactor (HTGR)
RES was visited by the Gas Cooled Reactor Associates (GCRA) General Manager and by DOE gas-cooled reactor division personnel July 22 to update NRC on the industry strategy plan and schedule for licensing a high temperature gas-cooled reactor (HTGR). The GCRA General Manager, Mr.
Dan Mears, outlined the strategy plan and presented NRC with a copy of their letter to CMB Director Stockman and DOE Secretary Edwards requesting support for the lead project initiative and funding to offset the 'first-of-a-kind" cost and risks associated with the HTGR.
DOE representatives have requested that NRC/RES consider development of a joint memorandum of understanding regarding conducting safety research for HTGR's. DOE will provide a draft agreement for RES review within a few I
weeks.
1 GCRA intends to provide RES, by September, a copy of their licensing plan for the HTGR plant. The strategy plan indicates the identification of lead project candidate utilities in FYB2 (already accomplished) and site l
selection in FYB3.
l EWR Full Intecral Simulation Test (FIST)
The contract modification to extend the BWR FIST program to the second phase has been sent by the Division of Contracts to the General Electric Company and the Electric Power Research Institute. This concludes approx-t imately 2 years of detailed planning and negotiations to define an integral EWR experimental program which meets the needs of all three program sponsors.
The FIST program has replaced the TLTA program, which was primarily for large i
break LOCA tests, with a program investigating a wide variety of BWR transients.
Testing is scheduled to start in November or December 19E2 and will extend through FY 1964.
Thermal Fluid Mixing Creare Incorporated is continuing its literature search for thermal fluid mixing models which can be used to describe heat transfer during pressurized thermal shock conditions.

The potential eligible party States that have been negotiating the compact are the New England States (Connecticut, Maine, Massachusetts, New Hampshire, Rhode Island and Vermont) and the Mid-Atlantic States (New Jersey, New York, Pennsylvania, Maryland and Delaware).
Coastal Zone Manacement State Programs, ELD and NRR staffs completed review and comment on the New York Coastal Zone Management Program and Draft Environmental Impact State-ment. The Program incorporates existing New York energy facility siting policy and regulation.

WEEK ENDING JULY 30, 1982 Southern California Edison Company San Onofre DN 50-361 The reactor achieved initial criticality at about 8:00 p.m. (PDT) on July 26, 1982.
On July 28, 1932, the Commission met to consider issuance of a full power license for this unit. Except for a representative of the Safeguards Branch, who attended in person, Region V representatives participated in the meeting by telephone.
At the conclusion of the meeting, the Commission voted to grant a full power license.
Portland General Electric Company Trojan - DN 50-344 On July 24, 1982, the licensee completed replacement of a section of the pressurizer surge line, following removal of the thermal sleeve located in this area. Following acceptance of the welds, the licensee commenced reloading the core, and as of July 30, 19S2, was about 70!; complete. The licensee plans to commence startup testing the week of August 16, 1982, with criticality expected later in the week.

Units 1, 2, and 3 were shut down--they all failed to pass the Tech Spec secondary containment integrity tests. Tests in the past gave credit for operational ventilation systems in other units; however, current tests do not consider these systems in evaluating the unit's integrity. Leaks in the containment system are being located and repaired in order to meet requirements.
Sumner, Unit 1 On August 4, the Atomic Safety and Licensing Board issued its decision on remaining contested issues and authorized NRR to issue a full-tem operating license for Unit 1.
The Board's favorable decision was subject to several conditions relating to seismic and emergency planning matters.
Indian Point Based on a letter from FEMA to the NRC, stating that radiological emergency response preparedness around the Indian Point reactor facility is inadequate, Region I sent a letter August 3 to Consolidated Edison and Power Authority of the State of New York inferming the licensee that if the deficiencies identified by FEMA are not corrected within 120 days, enforcement acticn will be considered.
Spent Fuel Storace On August 2 NRC met with the Nuclear Assurance Corporation (NAC) and dis-cussed licensing requirements for dry cask independent spent fuel storage installations (ISFSI). NAC is designing a cask which would meet NRC require-ments for spent fuel storage and off-site transportation.
LOFT A meeting of OECD countries is scheduled to take place in Paris on Septem-ber 20-21, 1982, to discuss the continuation of the LOFT program in the form of an international consortium.

Initial work will involve the placement of the essential cooling water pipeline in the ground.
Browns Ferry Nuclear Plant, Units 1, 2, & 3 Erowns Ferry Units 1, 2, & 3 all failed to pass the setendary containment integrity tests. At the time of the test on Unit 2, it was shut down for a refueling outa;e. Subsequently, Units 1 & 3 were tested and shut'down on Au;ust 3 and July 31, respectively, by the licensee because they did nct rest the Technical cification secondary containment integrity require ent.

When these tests were performed in the past on any ur$it, the ventilation systems of the other two units were maintained operational. The NRC inspectors determined that no credit should be taken for operation of the ventilation systems of the other units in evaluating the integrity the unit being tested because these systems may not be available following an accident (if offsite power is lust). As a result, all three BF plants failed to meet the Technical Specification requirement that the 0.25 inch negative water pressure be maintained in the secondary containment sys-tem. Only 0.1B inch negative water pressure could be achieved during the tests. Work is in progress to locate and repair leaks in the seccndary containment system to meet the Technical Specification requirement.

When the SDS becomes operational, approximately 40,000 gallons of reactor building sump water will be processed.

Core Inspection. One leadscrew on the core periphery and one leadscrew mioway from the periphery and the core center were removed from the TMI-2 reactor.
Initial attempts to remove a peripheral leadscrew (8P) were unsuccessful. The leadscrew could not be uncoupled from the control rod spider assembly. An alternate peripheral leadscrew (8B) on the opposite side of the core was eventually removed. Radiation levels in the proximity of both leadscrews were in excess of 50 R/hr. The maximum detected beta dose was 2000 Rad /hr. The-radiation levels in proximity of the center most leadscrew which was rer,oved on July 21, 1982, were less than 10 R/hr. Wet black crud was observed falling off the EB leadscrew as the leadscrew was being removed.
The closed circuit television inspection of the core periphery was limited to two spider assemblies (top of control rod end pieces). The camera could not be maneuvered under the spider assemblies and the condition of the fuel assemblies in that region of the core could not be ascertained.
The inspection of the core between the periphery and the center (control rod 9E) revealed a rubble bed approximately five feet below the top of the core region. This was the apprcximate depth of the rubble bed in the center of the core. Intact fuel pellets and pellet retaining springs were visible on tcp of the rubble.
Individual intact non-fuel bearing rods were seen protruding from the rubble bed toward the top of the core.
Some fuel assembly upper end fittings appeared attached to the underside of the plenum assembly. One end fitting was damaged. Fuel rod stubs protruded downwerd from some of the upper end fittings. There appeared to be some melting of structural materials in the area of the upper end fittings.
The procedure to probe the bottom of the rubble bed with a metal rod was deferred af ter the initial part of the inspection required more time tnan anticipated. The licensee is evaluating the inspection findings before deciding en the next course of action. The primary system remains depressurized and vented to the reactor building.
4 Gmundwater Monitorine. The recorded activity is based on the latest L a11acie analysis results of samples taken in Juf.e and July 1952.
Samples in the vicinity of the borated water storage tank (BWST) still snow tne increased tritium activity that apparently resulted from a BWST pipe leak which occurred during the winter when an instrument line froze and cracked. The maximum detected groundwater tritium concentration following the pipe leak was approximately one million picocuries per j
liter.
Water samples from test boring 2 in June and July 19B2, contained trace amounts cf cesium 137. The maximum cesium concentration detected during this period was 10 picocuries per liter. Small concentrations of cesium have teen detected periccically in test boring 2.

EPICOR II Prefilter Shioment. The first of 49 EPICOR 11 prefilters (PF-3) is scneculed for snipment to the Battelle Columbus Laboratory (BCL) on Aucust 11, 1982. The licensee is currently measuring hydrogen gas generation rates to demo'nstrate that nonflammable conditions will exist during all periods of PF-3 handling and transport. Gas measurements indicate that hydrogen will not exceed 3.5% and oxygen will be less than 1.0% during shipment. The licensee plans to inert both the PF liner and shipping task with nitrogen. gas as an added safety precaution. A type B shipping cask (designed to withstand transport I
accidents) will be provided by the Department of Energy for PF-3 transport to BCL. The licensee will demonstrate compliance with all l
applicable DOT and NRC transport requirements prior to shipment from the TMI site.

Purification Demineralizer Inspection. On August 3 and 6, 1982, the i
remotely operateo System In-Service Inspection (SISI) robot entered the auxiliary building purification demineralizer cubicles ("A" and "B") to visually monitor conditions and retrieve information on dose fields, loose surface contamination, temperatures on vessel walls, etc. The dose i
fields ranged from a general area of 2-5 R/hr at the doorway entering the I
cubicle to 1125 R/hr at approximately one foot from the bottom of the vessel. The cubicles were generally clean of debris and low levels of loose surf ace contamination were identified. # Boric acid residue was noted in the cubicles near the ficor drains. The temperatures on the external walls of the demineralizers were at ambient conditions (wS4*F).
The licensee is continuing preparations for measuring 3 conditions within the purification demineralizer vessels. These 100 ft stainless steel 1
vessels contain up to 50,000 curies of mixed fission products deposited on organic ion-exchange resins. Measurement of the internal vessel i
pressure is scheduled the week of August 9, 1982. Plans for gas and liquid sampling of the inlet and outlet lines is scheduled to begin in-September. The condition of the ion-exchange media (viz., sluicibility vs. non-fluid characteristics) within the vessels will dictate the l
cethodology used in the disposition of this highly radioactive waste.

Spent Fuel Storage On August 2,1982 NRL staff met with Nuclear Assurance Corporation (NAC) staff and discussed licensing requirements for dry cask independent spent fuel storage installations (ISFSI). NAC is designing a Storage / Transport (ST) cask which would meet requirements of 10 CFR Part 72 for spent fuel storage and 10 CFR Part 71 for off-site transportation.
CRBR Fuel Cycle Environmental Review As a result of the NRR decision to recirculate the draft supplement to the FES, the existing NMSS contract with EPNL (FIN No. B-2477) has to be extended to obtain additional support services. The contract was amended, changing tasks 3 and 4 to the following:
o Task 3: Respond to public comment and prepare draft section for EIS Final Supplement.
o Task 4: Provide support for the discovery process on the Final Supplement to EIS and public hearings.

facility which is a small (1/134 volume) integrated test facility simulating PWR. Four shakedown tests are being run with reflood-type

operating conditions. Most of NRC instruments are working satisfactorily even though these instruments were fabricated over three years ago and had been stored since then until late 1981.
Because of the TMI accident, the originally planned Core II large-break LOCA tests were postponed until a series of small break LOCA tests were completed in 1981.
When reflood-type shakedown tests are completed, a high pressure blowdown testing will connence, probably in September 1984.

To confim this safety analysis, it will be necessary to make a pre-test power ascension and compare fuel center-line temperatures with those numbered prior to the L2-5 test. i Code Review Group Meeting A Code Review Group meeting was held on July 27;4 Vuly 28,1982 at INEL.to review heat transfer medels in TRAC BWR and RELAP5, with emphasis on post-CHF and reflood modelling. Modelling for the effects of grid. spacers and flow blockage in COBRA-TF was also reviewed. The consensus was that the modelling suggested by Westinghouse for incorporation into COBRA.TF, may.be more comprehensive than is necessary.
Conse:;uently, some consultants agreed to suggest simpler alternatives.

Transoortation of Radioactive Material in South Carolina Transportation of radioactive materials into and within South Carolina was studied for the fourth and final year under a joint NRC/ DOT contract. The final report was issued this week as NUREG/CR-2842. The majority of the data presented results from inspections of shipments containing low-level radioactive wastes. The results indicate a significant decrease in the number of violations of State and federal regulations over the study period from October 1978 through September 1981. This may be attributed to the implementation of new legislation, increased efforts of field inspectors and strengthened enforcement activities.
Major recommendations are:

The Department of Transportation should increase the emphasis on carrier equipment inspections.
2.
Clarification and revisicn of Motor Carrier Safety regulations is needed and should include provisions for required, frequent l
carrier inspections of equipment.
3.
Carriers and drivers should be trained and knowledgeable as to their responsibilities and in applicable regulations.
Transportation of Radioactive Material in fievada A study by the State of Nevada between September 1980 and September 1981 concerning j
the transportation of radioactive materials became available this week as fiUREG/CR-2852. Data is presented on the surveillance of airport terminals, i
inspection of shipments of radioactive materials to the State disposal site, and i
on the transportation of radioactive materials to the largest user in the State disposal site, and on the transportation of radioactive materials to the largest user in the State, the Nevada Test Site. It was noted in the metropolitan areas thas taxicabs were used in the delivery of medical isotopes and the State has provided the cab companies with procedures to be followed. The major recorrnenda-tions resulting from the study were:
1.
Future State surveillance should include other hazardous materials; 2.
The U. S. Department of Transportation should tell the State the results of its enforcement; and 3.
The U. S. tJuclear Regulatory Commission should inspect all of its licensee's packaging of radioactive waste.

(1) for power reactors with a plant-specific simulator, continue the requirements of a simulator licensing exam of all new and replacement candidates and require, for the NRC-administered requalification exam, a simulator exam of at least 20% (per year) of the currently licensed operators, and (2) for power reactors without a plant-specific simulator, require an operating test (oral exams) in accordance with 10 CFR E55.23 as well as a written exam of all new and replacement candidates and require, for the NRC-administered requalification exams, oral and written exams of at least 20% (per year) of the currently licensed operators.
The Commission indicated that written exams are important in the assessment of operator qualifications and should not be deleted.

Prepare procedures that assure near equivalency between examinations at plants with and without simulators; and j

Prior to exeeding 5 percent power operation, the licensee must provide formal documentation of information regarding EVAC design fabrication and installation, discussed in meetings with the NRC on August 2-4, 1982.
2.
Pricr to exceeding 50 percent power operation, the licensee shall submit to the NRC the results of an independent review of the HVAC system,, including design changes, fabrication, and installation.
The review should encompass all safety-related HVAC systems and the effect of non-safety-related EVAC system failures on safety systems.
The conditions may be lifted by staff when the matter has been resolved to their satisfaction.
Further, the Commission's authorization is given with the understanding that the Director, NRR, will act on the remaining 2.206 petition before issuing the license amendment.
