Licensed Operator Biennial Medical Examination Your letter of August 4, 1993, which transmitted NRC Inspection Report Nos.

282/93010(DRP) and 306/93010(DRP), requested a response to a violation. The following is offered in response to the violation.

10 CFR Part 55.53(i) states that, "The licensee shall have a biennial

corrective actions were communicated to Region III as documented in Inspection Report Nos. 282/93010(DRP) and 306/93010(DRP).

These discrepancies included l

the instance of a licensed operator whose medical examination interval had i

exceeded the allowable interval by two years and two instances in which the medical status of licensed operators had changed (to require corrective lenses) without notifying the NRC per 10 CFR 55.25.

Revised NRC Forms 396 i

were submitted for these two operators.

Since then we have discovered another instance in which the medical status of a licensed operator had changed (to require corrective lenses) without notifying the NRC per 10 CFR 55.25. lua NRC Form 396 for this operator is being submitted to the Regional Administrator i

under a separate letter.

Reasons for Violation l

l The violation occurred for several reasons:

1 Inadequate communication between the corporate medical department and j

training center personnel responsible for scheduling medical j

examinations.

r Inadequate detail in procedures for tracking, scheduling, and reporting of medical examination status.

Inadequate supervisory review and oversight of the medical examination e

e process.

4 Corrective Steps Taken and Results Achieved l

A review of the medical records was performed to determine compliance with the regulations and the results are discussed above and in Inspection Report Nos, j

282/93010(DRP) and 306/93010(DRP).

The subject operator received a medical examination on April 1, 1993.

The operator was determined to be fully qualified.

Corrective Steps to Avoid Further Violations i

J The procedure for license applications, renewals, and medical examinations has been revised to identify responsibil tles for tracking, scheduling, and reporting of medical examination status.

The procedure includes supervisory responsibility for review of the process and results.

i This violation will be discussed with all licensed individuals to convey their responsibility for ensuring the required medical examinations are obtained.

j This will be completed by September 24, 1993.

In addition, an internal audit (AC 93-27-20, Medical Evaluations) is currently being performed by the Quality department. Any program deficiencies (beyond l

those already identified and corrected for) identified by this audit will be corrected in accordance with our Corrective Action program as appropriate.
