Immediate Action Letter 80-55 Gentlemen:

Pursuant to a telecon between Mr. L. E. Tripp of "nur office and our Mr. W, P. Johnson, Vice President, Yankee Atomic Electric Company, and as required by I= mediate Action Letter (IAL) 80-55, we have taken the following specific, positive actions applicable to repair welding.

1.

Effective 12:00 noon, December 22, 1980, Pullman-Higgins, as directed, ceased all safety-related repair welding.

All process sheets for repair welding were placed on hold. The suspension of welding repair activities was verified by Yankee Atomic Electric Company Field Quality Assurance personnel.

2.

Pullman-Higgins has initiated additional positive measures to assure that the correct welding parameters and inspection are specified l

prior to repair welding.

These measures include major revisions to Pullman-Higgins Job Specification JS-IX-14, " Defect Removal and Repair by Welding," to provide for more effective controls over the entire repair welding process.

Revisions will include, but not be limitt 0,

the following:

1)

Developing a dual repair process sheet system which l

dictates more controls during the excavation and welding l

processes.

The first system is dedicated to excavation, dimensional characteristics, and evaluation.of the affected area.

l Based upon the results of the evaluation, a second process sheet for repair welding will be developed specifically to the characteristics of the affected area.

2)

Additional verification hold points will also be included to assure that the correct weld procedures and inspection criteria are selected and properly implemented.

Verification of software is also now required subsequent to the repair process to assure that the completed repair records do in fact reflect the actual repair conditions.

b)

Pullman-Higgins has set a target date of January 9,1981 to accomplish the aforementioned.

In addition, Pullman-Higgins will re-review all previous documentation on completed weld repairs. This is to be completed by January 16, 1981.

3 Pullman-Higgins is presently developing lesson plans to provide more comprehensive training to appropriate site personnel.

Pull =an will have their lesson plans completed by January 9, 1981.

Training will include, as a minimum, the criteria associated with the repair welding procedure specification and the applicable NDE requirements.

Contractor groups presently scheduled for training are:

Quality Assurance Engineering, Engineering Staff, Field Quality Control (Inspection), Craft Supervisors and Superintendents, Field Engineering, and Welding Foremen.

Training of these key personnel will be completed by January 16, 1981.

Measures presently taken and those planned to increase Yankee Atomics' quality assurance involvement over Pullman-Higgins' repair welding activities include the following:

a)

Prior to the issuance of IAL 80-55, Yankee Atomic Field Quality Assurance personnel had initiated and were in the process of making a 100% review of Pull =an-Higgins ' repair welding process

sheets, b)

Yankee Atomic Field Quality Assurance personnel will continue a 100% review of Pullman-Higgins' present and proposed revised weld repair process sheet system. Surveillance of all repair welding activities will be continued by YAEC Field Quality Assurance personnel until such time as a =arked improved level of confidence has been established in Pullman-Higgins' perfor=ance.

Yankee Atomic audit schedules are present1y being revised to provide greater coverage over Pullman-Higgins' controls of their repair welding operations along with all other areas of their welding progyam.

Based upon the aforementioned r.easures of corrective action and contingent upon Yankee Atomic Electric Company's verificatio?.nat the systems and controls proposed are meaningfully and effectively implemented by January 16, 1

1981, we request that wc be permitted to remove the stop work order issued on repair welding oper: Lions by Pullman-Higgins.

In addition to the specific actions identified above, Yankee Atomic Electric Company has initiated the following longer-term generic actions.

Our audit and su.veillance plans are presently being reviewed and where applicable will be revised to include more frequent analysis and verification of contractors' and constructors' nonconformance and corrective acticn programs.

In addition, we are planning to assign a Quality Assurance Engineer who will be dedicated to the task of assuring the effectiveness of corrective actions taken project-wide.

He will be required to analyze all project nonconformance reports for generic implications as well as specific discipline or contractor ccrrective actions. The assigned incividual will also be responsible for assuring that meaningful consideration is taken during Project Nonconformance Review Board Meetings regarding the evaluation of nonconformances as they relate to 10CFR50 55(e) or 10CFR Part 21 requirements.

The Quality Assurance Engineer dedicated to the task of assuring positive and effective corrective action will also be responsible for assuring that NRC inspection and internal audit findings are evaluated on a generic basis anu not merely for the specific finding.

Yankee At omic Electric Company Quality Assurance personnel, both Home Office and Field, attended a Project Quality Assurance Seminar on Januacy 6, 1981.

The topic of broad range generic aspects related to audit, surveillance, and NRC-identified items of concern or noncompliances and their corrective actions was discussed and re-empSasized.
