27 07/28/2017 L-2017-CRS-0048 RIS Electronic Signature Use for Medical Licensee Internal Documents Required by NRC Regulation This RIS is intended to supply information to assist medical use licensees in complying with the current NRC requirements related to the use of electronic signatures in internal licensee electronic documents and to clarify what constitutes a signature on electronic documents that medical use licensees are required to sign and maintain.

26 08/24/2017 L-2018-GEN-0005 RIS Pre-Application Communication and Scheduling for Accident Tolerant Fuel Licensing Submittals The staff has generated a list of questions regarding anticipated agency work in the area of accident tolerant fuels (ATF). This RIS will provide the venue for collecting that information.

15 07/30/2018 L-2018-GEN-0012 IN Operating Experience Regarding Flow-Accelerated Corrosion Flow-accelerated corrosion (FAC) is an industry issue that was first recognized to be of significant concern at Surry in 1986. There continue to be FAC failures attributed to problems with the FAC monitoring program and its software that have caused reactor trips.

5/30/2019 L-2019-GEN-0010 IN - Strontium-82/Rubidium-82 Generator Breakthrough Events and Issues The purpose of this IN is to inform addressees of incidents where excessive levels of Strontium-82 and Strontium-85 were unknowingly administered to patients during a cardiac imaging procedure because of failed Strontium-82/Rubidium-82 generators.

3 08/2019 L-2019-GEN-0011 RIS - Clarification of Two-Person Rule for Industrial Radiography The U.S. Nuclear Regulatory Commission (NRC) is issuing this regulatory information summary (RIS) to provide an alternative regulatory interpretation of NRC surveillance requirements to allow the second qualified radiographer to perform other tasks in another area while still being able to meet the surveillance requirement to provide immediate assistance.

Medical events described in this IN are attributed to written directive errors, clogging or kinking of the delivery system, and treatment of the wrong location.

This information will help the NRC plan its resources more effectively.

L2019-GEN-0000 IN Effective Cyber Security Practices for Consideration to Protect Digital Assets of Byproduct Materials Licensees The purpose of this IN is to inform affected byproduct materials licensees of the results of an evaluation that was previously conducted by NRC staff of the potential need for cyber security requirements and of effective cyber security practices for licensees consideration. The INs enclosure will include the effective practices that provide the addressees information on contemporary cyber security issues related to the protection of digital assets, including those assets used to facilitate compliance with physical security requirements such as Title 10 of the Code of Federal Regulations (10 CFR)

Part 37, Physical Protection of Category 1 and Category 2 Quantities of Radioactive Material.

01/15/2019 08/14/2019 L-2019-GEN-0006 IN Potential Over-Pressurization of High Specific-Activity Alpha-Emitting Radioactivity Sources A liquid source containing 1.5 mCi (55.5 MBq) of Americium 241 was stored in a sealed glass storage container for 12 years, which burst in August 2017, causing significant personnel exposure risk. The cause of this container failure is believed to be the generation of hydrogen gas through hydrolysis, which caused pressure buildup in the container, and which is likely to be a problem for similar high specific-activity alpha-emitting radionuclides in an aqueous solution.

01/15/2019 08/26/2019 L-2019-GEN-0004 IN Patient Skin Contamination Events Associated with I-131 Mibg During Neuroblastoma Treatments The information notice alerts licensees to the potential patient contamination risks associated with therapeutic treatments with iodine-131 meta-iodobenzylguanidine (I-131 Mibg). The NRC expects that licensees will review the information for applicability to their facilities and consider actions, as appropriate, to avoid similar problems.
