1. Results and assessment of review of IMC 0308, ROP basis document review IP 71111.12 was last issued on February 3, 2016. Review complete, no issues noted.

Verbiage in the bases section of the IP will be removed and a reference to IMC 0308 will be incorporated.

2. Results and assessment of review of any applicable changes to PIs A three year review by the PI lead did not identify any significant changes to PIs that would result in a reduction or unintended gap in the key safety attributes of each safety cornerstone.
3. Results and assessment of review of any applicable changes to Rules and STSs Revisions to rules and Standard Technical Specifications within the last three years did not indicate a need for revision.
4. Results and assessment of review of recent Operating Experience Operating Experience Note 2016-1: Discusses four events between August and December 2015 involving electrical connection fit-up issues that could lead to personnel injury, damage to safety-related equipment, and signifi-cant plant transients. The note recommended inspectors verify during maintenance activities verify craft reliance on labeling and plant procedures and that pre-job briefs include a discussion of applicable operating experience and hold points when anomalies are encountered.

Operating Experience Note 2016-7: Discusses Issues with qualifying Electroswitch power switches and relays and the Vendors Part 21 Program. The note recommended that inspectors verify that applicable sites have the Electroswitch Part 21 issues are entered into the Corrective Action Program and non-applicable plants, have inspectors review the CAP or Operating Experience program for the issue.

Operating Experience Note 2017-1: Discusses a steadily increasing failure rate for DC power systems related to system aging and inadequate maintenance related to batteries, chargers, motor-generators, and inverters as well as subcomponents including breakers, circuit cards, control panels and switches. The note recommends the inspectors consider including DC Power as a sample selection during Maintenance Effectiveness inspections Operating Experience Note 2018-1: Discusses operator workarounds and inadequate application of vendor guidance for properly measuring oil level produced bearing damage, which was attributed to a lack of lubrication caused by insufficient level in the oil reservoir. The note recommended inspectors look for leaks and improperly installed sight glasses. Also, this note discusses EDG voltage regulator diode failures at Catawba from thermal aging and Wolf Creek by overloading and overheating. The note recommended inspectors to verify that licensees 1)

2 follow up on operating experience regarding the reliability of safety-related components and 2) take timely corrective action when they discover equipment that is at or near end of life.

A Feedback Form may be initiated to reference these Operating Experience notes.

5. Results and assessment of review based on RRPS data (see separate IP 71111.12 Assessment Data file for details of RRPS data)

Adverse trends or outliers noted: No Assessment:

o From 2015 through 2017, the total number of Findings decreased by 33.7 percent (page 1 of 10 of IP 28). Since IP inception, the trend line depicts a slight increase in Findings per year. However, since 2013, the trend line shows a modest decrease in Findings per year.

o For 3 Year Findings/1000 hrs, Region 4 had the most at 8.0 and Region 2 had the least at 5.9 (page 3 of 10 of IP 28). There is not a huge difference between the regions.

o From 2015 to 2017, the average hours per sample among the four regions is 9.4, where Region 4 had the highest at 10.6 and Region 2 had the lowest at 8.6. The Budgeted range for IP 71111.12 is 84 to 102 hours0.00118 days <br />0.0283 hours <br />1.686508e-4 weeks <br />3.8811e-5 months <br /> and 8 to 10 samples per site per year. Average hour/sample/site is 10.4. It appears that the actual hours charged are within the budgeted amount and no adjustments are required.

6. Feedback Forms FBFs Closed Last three years Approved Feedback Form 71111.12-2061 (ML16033A367) recommended incorporating inspection guidance regarding when and how to specifically assess age-related degradation issues at plants and was the only FBF closed in the last three years.

Open FBFs Currently, Feedback Form 71111.12-2325 which recommends eliminating vertical slice inspections, remains open

8. Results of discussions with regions On Monday October 1st, the results of the assessment were discussed with the regions, no changes to the inspection procedure was recommended.
