Failure of Channel I of the Pressurizer Overpressure Protection System (POPS)

CONDITIONS PRIOR TO OCCURRENCE:

Cooldown Operations Mode 4 0% Power DESCRIPTION OF OCCURRENCE:

While testing the Pressurizer Overpressure Protection System prior to placing the system in service, as required by Technical Specification 3.4.9.3, Channel I relief valve would not open.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure ANALYSIS OF OCCURRENCE:

Two POPS relief valves or a Reactor Coolant System vent of at least 3.14 square inches are required to be operable when the Reactor Coolant cold leg temperature is less than or equal to 312°F.

A broken open limit switch on POPS relief valve 2PR47 prevented the valve control circuitry from operating properly resulting in the limiting condition for operation being exceeded until the Reactor Coolant System could be cooled down, depressurized and vented as required by the Action Statement.

CORRECTIVE ACTION

The limit switch assembly was replaced and adjusted.

The high failure rate for this type limit switch has been previously noted (LER 80-05/03L).

There have been three limit switch failures since the present type limit switches were installed on both POPS relief valves on May 22, 1980.

LER 80-29/03L 2 -

Design Change Request 2SC-0397 was initiated on June 26, 1980, to investigate and specify improved switches.

FAILURE DATA:

Marotta Scientific Controls Valve Model MV-225C This is the third limit switch failure since May 22, 1980.
