• NRC-identified Green Non-Cited Violation of Technical Specification 5.4, Procedures was identified because Entergy staff did not implement procedure AP-12.06, Equipment Status Control, as required. Specifically, Entergy personnel did not recognize the impact of a change associated with the tagout of 10MOV-13C, the C Residual Heat Removal system suction containment isolation valve.

• Created reasonable doubt of operability, which was unrecognized. This resulted in failure to declare the PCIV inoperable and enter TS 3.6.1.3.
