Prairie Island Public Meeting Substantive Cross-Cutting Issues in the Area of Human Performance Red Wing, MN December 14, 2010

Purpose of Todays Meeting The NRC asked the company to present to NRC and the public its actions to address the substantive Human Performance issues that continue to exist since they were first identified in mid 2009 in mid 2009 The NRC asked the company to discuss how they have addressed human performance issues, improvements made and actions still needed to resolve the issue.

NRC Performance Goals Safety Ensure adequate protection of public health and safety and the environment Security Ensure adequate protection in the secure use and management of radioactive materials

Reactor Oversight Process 3 Strategic Performance Areas are divided into 7 Cornerstones of Safety Inspection Findings and Performance Indicators are assigned to a Cornerstone Inspection Findings can be assigned a cross-cutting aspect (a causal factor for the issue)

Human Performance Problem Identification and Resolution Safety Conscious Work Environment

There are 104 Operating Reactors in the United States 86 Licensee Response Column - require no additional oversight Reactor Oversight Process g

10 Regulatory Response Column - increase in oversight 8 Degraded Column - more oversight 0 Multiple Degraded Column - high level of oversight Of the 104 Reactors, 14 have Substantive Cross-Cutting Issues

Current Prairie Island Assessment Unit 1 is in the Licensee Response Column (Column I)

Unit 2 transitioned from the Regulatory Response Column (Column II) back to Column I on November 24, 2010 after successfully completing an inspection for a previous White Finding which involved the failure to adequately protect a safety system from damage

t Aspects of performance which demonstrate attitudes or behaviors that affect many areas of the plant A major contributing factor causing the issue NOT violations of NRC requirements Receive significant NRC attention and must be addressed

When four or more findings share a common cross-cutting aspect in a year NRC identifies it as a trend NRC determines if the plant is taking What is a Substantive Cross-Cutting Issue (SCCI)?

For the 2010 Mid-Year Assessment, the NRC identified an SCCI in the Human performance area Substantive Cross-Cutting Issue at Prairie Island Human Performance Systematic Process Procedural Accuracy Procedural Compliance

Plant has had human performance issues for over 1 year Plant has taken actions to address these Human Performance issues NRC has seen improvement based on:

Substantive Cross-Cutting Issue at Prairie Island - What We See Fewer violations, items with cross-cutting aspects People who operate and maintain the plant are focusing on actions to enhance their performance Observations by NRC inspectors who have noted plant personnel placing better focus on their actions NRC still has concerns Sustainability

Conducted additional meetings with the licensee Reviewed the independent safety culture assessment of the plant Will ti t

th NRCs Actions Will continue to focus inspections in the area of Human Performance Will perform a comprehensive review of human performance issues and document in the NRCs assessment of plant performance for year 2010

The cross-cutting issue will be resolved when the number of findings with the same cross-cutting aspect goes down and the NRC has confidence that improvement Resolution of Cross-Cutting Issues the NRC has confidence that improvement will be sustained going forward
