One notable event occurred on November 21, 2009, when a small amount of radioactive material inadvertently became airborne within the reactor building containment during OTSG replacement activities. Consequently, several workers became contaminated and/or ingested small amounts of radioactive material. This issue is the subject of a separate NRC ins pectio nactivity.
