PSEG Nuclear, LLC (PSEG) hereby submits its Steam Generator Tube Inspection Report consistent with the requirements of Technical Specification (TS) 6.9.1.10. The report is being submitted within 180 days after the initial entry into HOT SHUTDOWN following completion of the inspection performed in accordance with Technical Specification 6.8.4.i, "Steam Generator (SG) Program." Box 415 Trenton, NJ 08625 Document Control Desk LR-N07-0265 PSEG Nuclear LLC Salem Unit 1 STEAM GENERATOR TUBE INSPECTION REPORT TS 6.9.1.10 Introduction In 1998 Salem Unit 1 commenced operating cycle 13 with the newly replaced steam generators (SG) from the canceled Seabrook-2 plant. Salem Unit 1 is a four (4) loop Westinghouse Pressurized Water Reactor (PWR) with Model F SGs that incorporate thermally treated Inconel 600 U-tubes. The tube bundle consists of 5,626 tubes with an outside diameter (OD) of 0.688 inches and a nominal tube wall thickness of 0.040 inches. During SG fabrication, the tubes were hydraulically expanded over the full tubesheet depth and the first ten tube rows were thermally stress relieved after U-bend formation. The seven (7) tube support plates (TSP) are fabricated from 405 stainless steel (SS) and have broached "quatrefoil" tube holes. A flow distribution baffle (FDB) plate, also 405 SS, between the secondary face of the tubesheet and the first tube support plate, provides top-of-tubesheet (TTS) sludge pile control. Tube support within the U-bend region is provided by three V-shaped chrome-plated Inconel 600 antivibration bars (AVB's). provides a general summary of the Salem Unit 1 Model F SG Tube Support Arrangement and Terminology.

Consistent with Technical Specification (TS) 6.9.1.10, this report is being submitted within 180 days after the initial entry into HOT SHUTDOWN following completion of inspection performed in accordance with the Specification 6.8.4.i, "Steam Generator (SG) Program." This report includes:

a.

The scope of inspections performed on each SG,

b.

Active degradation mechanisms found,

c.

Nondestructive examination techniques (NDE) utilized for each degradation mechanism,

d.

Location, orientation (if linear), and measured sizes (if available) of service induced indications,

e.

Number of tubes plugged during the inspection outage for each active degradation mechanism,

f.

Total number and percentage of tubes plugged to date, and

g.

The results of condition monitoring, including the results of tube pulls and in-situ testing.

h.

The number of indications detected in the upper 17 inches of the tubesheet thickness along with their location, measured size, orientation, and whether the indication initiated on the primary or secondary side.

The operational primary to secondary leakage rate observed in each steam generator during the cycle preceding the inspection and the calculated accident leakage rate for each steam generator from thelower most 4 inches of tubing (the tubesheet is nominally 21.03 inches thick) for the most limiting accident. If the calculated leak rate is less than 2 times the total observed operational leakage rate, the 180 day report should describe how the calculated leak rate is determined.

Page 1 of 5 Document Control Desk LR-N07-0265 SG inspections were performed in accordance with TS 6.8.4.i, "Steam Generator Program", during Salem Unit 1 Outage 1R18 (April 2007). Each reporting requirement of the U1 TS 6.9.1.10 is addressed below (items a through i).

Tech Spec 6.9.1.10.a The scope of inspections performed on each SG are summarized below:

" A full-length bobbin coil inspection was performed on 100% of the in-service tubes in each steam generator.

" Rotating coil (+ Point) on all hydraulic over expansions (OXP) at the hot and cold leg top of tubesheet.

  • Rotating coil (+ Point) on 20% of the total Hot Leg (HL) population of OEX (Ž1.5 mils) and BLG

(Ž_18 volts) tubesheet expansion anomalies that are between the TTS to -17" below the TTS. The tubesheet drilling process during manufacture can create minor irregularities in the tubesheet boreholes. When the tube is expanded into the borehole, including into the minor irregularities, the tube wall's variance from a straight normal cylindrical shape is typically referred to as an expansion anomaly. These are classified as an internal tubesheet over expansion (OEX, typically measured in mils) or as a bulge (BLG, typically measured in bobbin probe voltage).

" The following was conservatively performed in approximately 62 tubes from all four SGs as identified from previous efforts supporting resolution of issues stemming from OE 13898:

" Performed rotating coil +Point inspections of>5 volt dents and dings (HL and CL)

" Performed rotating coil +Point inspection of => 2 volt u-bend dents and dings

" Performed a rotating coil +Point inspection of AVB wear that was reported during the bobbin coil inspection

" All previously installed tube plugs, and plugs installed during 1R18, were visually inspected Note that a ding at a support structure (eg: TSP, AVB) is called a Dent (DNT) at Salem Unit 1.

Therefore, the only difference between a Dent and a Ding is the location within the freespan with respect to a support structure.

Tech Spec 6.9.1.10.b Active degradation mechanisms found:

The only active degradation mechanism detected during outage 1R18 was AVB wear. A summary of the AVB wear indications is provided in the table below, and in Attachment 4.

In accordance with Rev. 6 of the EPRI PWR Steam Generator Examination Guidelines (section 6.2.4) a documented review of EPRI Qualified techniques was performed to ensure application of these techniques are pertinent to site-specific SG conditions. See Attachment 3 for examination techniques utilized for each degradation mechanism. The region of tube, to which these techniques are applicable, includes both those that were specifically demonstrated in the industry qualifications and others for which there is no specific demonstration (extended).

Tech Spec 6.9.1.10.d Location, orientation (if linear), and measured sizes (if available) of service induced indications:

The only service induced indications detected during outage 1 RI 8 were due to AVB wear. Attachment 4 provides information for these indications.

Tech Spec 6.9.1.10.e Number of tubes plugged during the inspection outage for each active degradation mechanism:

AVB wear is the primary degradation mechanism for the Salem Ul Model F SGs, and the only active degradation mechanism detected during 1R18. Other than AVB wear, the only other tube plugging was preventative plugging for one tube in SG12 (tube R5-C31) for Permeability Variation (PVN).

  • Note that AVB wear indications in all four SGs were conservatively plugged at 33% TW, therefore the largest in-service AVB wear indication is 32%. This is discussed more in response to 6.9.1.10.g.
    • Permeability Variation is a condition where the test coil impedance changes due to a change in the tubing material's inherent willingness to conduct magnetic flux lines. Permeability variation signals are not considered to be flaws or degradation. Rather, the signals are considered data quality issues that have additional testing using a magnetically biased rotating coil probe to disposition. The results of this testing did not reveal any flaw like response indicative of degradation in the area of interest. PSEG made the conservative decision to preventively plug this tube. It is PSEG's position that any flaws or degradation in the area where PVN was reported would result in a change in signal response when compared to previous inspection data. Based on look backs performed, there were no observed changes in the signals.

Following the completion of outage 1 R18, a total of 96 additional tubes were removed from service from all four-steam generators. This resulted in a total of 224 tubes, or approximately 0.995%, removed from service from all four steam generators. The limiting steam generator is SG 13 with approximately 1.831%

of tubes removed from service. The Table below provides further details.

Tech Spec 6.9.1.10.g The results of condition monitoring, including the results of tube pulls and in-situ testing:

A Condition Monitoring (CM) assessment was performed for the Salem Ul SGs following inspections from outage 1R18 in accordance with TS 6.8.4.i, NEI 97-06, and EPRI Guidelines (including the EPRI Steam Generator Integrity Assessment Guidelines, Revision 2). Consistent with TS 6.8.4.i.a, "Provisions for condition monitoring assessments", "Condition monitoring assessment means an evaluation of the "as found" condition of the tubing with respect to the performance criteria for structural integrity and accident induced leakage. The "as found" condition refers to the condition of the tubing during an SG inspection outage, as determined from the inservice inspection results or by other means, prior to the plugging of tubes. Condition monitoring assessments shall be conducted during each outage during which the SG tubes are inspected or plugged to confirm that the performance criteria are being met" The "as found" condition of the tubing for 1RI 8 is provided in response to TS 6.9.1.10.d.

The Salem Unit 1 Condition Monitoring Limit (CML) for AVB wear is approximately 63% Through-Wall (TW), when using EPRI ETSS technique 96004.1, and as conservatively calculated using guidance consistent with the EPRI Steam Generator Integrity Assessment Guidelines, Revision 2. AVB wear measured less than the CML analytically demonstrates that the structural integrity performance criterion (TS 6.8.4.i.b. 1) is met. Condition Monitoring can be accomplished by analytical methods or in situ pressure testing. All AVB wear indications detected during IR1 8 were found to be well below the NDE analytical CML, except for an indication in SG13 (tube R54-C65 at AV3). The indication in SG 13 R54-C65 was sized at 71%TW with bobbin probe technique 96004.1. Therefore, analytical demonstration of this AVB wear indication could not be accomplished. Because it could not be demonstrated analytically with NDE, it was necessary to perform in-situ pressure testing. In-situ pressure testing was performed on this indication (full length in-situ of this tube), for proof and leak testing in accordance with the EPRI Steam Generator In Situ Pressure Test Guidelines, Revision 2. The results of the pressure test did not provide any leakage at any pressure hold points, up to the three times the normal operating differential Page 4 of 5 Document Control Desk LR-N07-0265 pressure equivalent as provided by the requirements of the EPRI SG In-Situ Pressure Test Guidelines (approximately 4950 psi). Therefore, structural and leakage integrity was confirmed. This tube was plugged and stabilized during IR18.

Of all the AVB wear indications detected during 1R18, the indication in SG13 (tube R54-C65 at AV3) was the only AVB wear indication of this severity. The next two largest AVB wear indications are 53 and 52% TW, with the majority of the AVB wear less than 40% TW. Tube plugging in accordance with the Tech Spec repair criteria (40% TW and greater) provides that 26 tubes require tube plugging for AVB wear. However, the plugging limit for AVB wear was lowered from 40% TW to 33% TW, to support the Operational Assessment and provide margin. Therefore the largest AVB wear returned to service is 32%

TW.

None of the indications detected during the 1 R18 inspection challenged the structural integrity performance criterion; therefore the accident induced leakage performance criterion (TS 6.8.4.i.b.2) is also satisfied. This was also directly demonstrated by the in-situ pressure test performed for the most limiting AVB wear indication detected during 1 R18.

No SG tube pulls were performed during Refueling Outage 18.

Tech Spec 6.9.1.10.h The number of indications detected in the upper 17 inches of the tubesheet thickness along with their location, measured size, orientation, and whether the indication initiated on the primary or secondary side:

There were no indications of degradation detected in the upper 17 inches of the tubesheet thickness during the IR1 8 outage inspections.

Tech Spec 6.9.1.10.i The operational primary to secondary leakage rate observed in each steam generator during the cycle preceding the inspection and the calculated accident leakage rate for each steam generator from the lowermost 4 inches of tubing (the tubesheet is nominally 21.03 inches thick) for the most limiting accident. If the calculated leak rate is less than 2 times the total observed operational leakage rate, the 180-day report should describe how the calculated leak rate is determined:

Salem Unit 1 did not observe any primary to secondary operational leakage at or greater than the detection threshold of approximately one (1) gallon per day over the cycle preceding the inspection in any of the steam generators. Since Salem Unit 1 did not observe any significant measurable SG primary to secondary operational leakage, the calculated accident leakage rate for each steam generator from the lowermost 4 inches of tubing for the most limiting accident would also not be significant. Conservatively assuming that there was a primary to secondary operational leakage at the detection threshold of approximately one (1) gallon per day also provides that the estimated calculated accident leakage would also be negligible.
