REVIEW OF ARIZONA PUBLIC SERVICE COMPANY LETTERS DATED DECEMBER 8, 2006, INVOLVING CORRECTIVE ACTIONS FOR SUBSTANTIVE CROSSCUTTING ISSUES
Dear Mr Levine:
Thank you for your two letters dated December 8, 2006, involving your corrective actions for substantive crosscutting issues in human performance and problem identification and resolution. As discussed during a public meeting on September 7, 2006, the substantive crosscutting issues have existed for the previous two years, demonstrating that your implementation of prior corrective actions has not been successful in addressing performance concerns. This conclusion is based on the continued identification of numerous findings by NRC inspectors and periodic NRC assessments which determined that the implementation of integrated improvement plan corrective actions for the crosscutting areas was ineffective.
The NRC will continue to monitor human performance and problem identification and resolution activities as part of the baseline inspection program. Additionally, the NRC will perform a closure inspection of these substantive crosscutting issues following written notification from the Arizona Public Service Company that you have completed an assessment of the crosscutting areas, implemented additional actions needed to improve performance, and that measures and metrics have shown the desired results to support closure of the crosscutting areas. Elements of the closure inspection scope will include an assessment of the actions described in your December 8, 2006, letters and actions described in your integrated improvement plan.
