REPRINT OF NRC FORM 396 Enclosed are several copies of NRC Form 396, Certification of Medical Examination by Facility Licensee, which was reprinted in December 2005. Additional copies may be obtained by calling Mr. Chris Colburn, in NRC Forms Management, Office of the Chief Information Officer, at 301-415-6157, or, if you prefer, by electronic mailing him at FORMS@NRC.gov.
NRC Form 396 is available in *.pdf format from the NRCs Operator Licensing web site at http://www.nrc.gov/reading-rm/doc-collections/forms/nrc396.pdf.
In accordance with 10 CFR 2.390 of the NRC's "Rules of Practice," a copy of this letter will be available electronically for public inspection in the NRCs Public Document Room or from the Publicly Available Records (PARS) component of the NRC's Agencywide Documents Access and Management System (ADAMS). ADAMS is accessible from the Electronic Reading Room page of the NRCs public Web site at http://www.nrc.gov/reading-rm/adams.html.
