ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a Notice of Violation associated with a White Significance Determination Process (SDP) finding will be issued on or around March 05, 2004, to FirstEnergy Nuclear Operating Company as a result of inspections at the Davis Besse Plant. This White finding, an issue with low to moderate increased importance to safety, involved the failure to correctly design the high pressure injection (HPI) pumps for accident mitigation. The licensee identified a design deficiency regarding internal clearances of the HPI pumps which could have rendered the pumps unable to inject water into the reactor coolant system during the recirculation phase of emergency core cooling following a loss of coolant accident. During this mode of operation the HPI pumps were susceptible to failure since the potential existed for debris from the sump to be transported to the HPI pumps and cause blockage of lubricating water to the pumps hydrostatic bearing resulting in a loss of the HPI pumps due to excessive vibration or overheating or both.
The failure to adequately implement design control measures for verifying and checking the adequacy of the original design of the HPI pumps is a violation of 10 CFR 50, Appendix B, Criterion III, "Design Control." Accordingly, a Notice of Violation associated with the White finding is being issued.
It should be noted that the licensee has not been specifically informed of the enforcement action.
