Subject:
ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a Notice of Violation associated with a White Significance Determination Process (SDP) finding will be issued on or around December 29, 2003, to Entergy Operations, Inc. as a result of inspections at the River Bend Station. This White finding, an issue with low to moderate increased importance to safety, involved the failure to properly lock open condensate prefilter vessel bypass flow control Valve CNM-FCV200 resulting in a loss of feed water flow to the reactor on September 18, 2002, when the valve unexpectedly closed following a reactor scram. The failure to properly lock open the valve is a violation of Technical Specification 5.4.1.a. Accordingly, a Notice of Violation associated with the White finding is being issued.
It should be noted that the licensee has not been specifically informed of the enforcement action. The schedule of issuance and notification is:
