Dear Sir or Madam:

This letter submits the James A. FitzPatrick Nuclear Power Plant (JAF) Owner's Activity Report (OAR-1), Third Period, Fifth Interval which contains a summary of the lnservice Inspection (ISI) examinations conducted during the 2024 Refuel Outage 26 (J1R26).

Table 1 of the enclosed report summarizes the items with flaws or relevant conditions that required evaluation for continued service and Table 2 provides an abstract of repair/replacement activities required for continued service.

This report is being submitted in accordance with JAFs approved Fifth Interval Inservice Inspection Program Plan [Reference 1].

ASME Section XI Code Case N-513-4 was employed.

IR 04702273, Op Eval 639783 F-A, F1.30c PFSK-4265, Variable spring support VT-3 examination of variable support PFSK-4265 identified that the cold load setting was out of tolerance and a loose nut was observed. The as found condition was evaluated per EC 642393 and it was determined the support can perform the intended design function in the as found condition.

IR 4800448, EC 642393 E-A, E1.12 Wetted Surfaces of Submerged Areas.

VT-3 visual examination of the torus shell plates for bays M, B, P, H, and N. The inspection identified pressure retaining components corrosion or erosion that exceed 10% of the nominal wall thickness at James A.

FitzPatrick. EC 642445 evaluated the as found condition as acceptable for continued service.
