iv' I Temporary Job Site Inspection 15450 Northline Road, Southgate, MI PROGRAM SCOPE 4. TELEPHONE NUMBER (419) 251-3700 NIA This was an unannounced inspection of a mobile nuclear medicine service based in Toledo, Ohio that was performing work under reciprocity at a temporary job site in Southgate, Michigan. The licensee provided nuclear stress test services to its client approximately once per month. A licensed radiopharmacy delivered unit doses of Tc-99M to the licensee's main office in Toledo. The nuclear medicine technologist (NMT) then transported all radioactive material to their client's location to perform the administrations. The client provided dedicated space for the licensee to perfonn stress test The inspector toured the area where administrations were performed and evaluated the licensee's measures for material security, hazard communication, and exposure control. The two licensee personnel on site maintained constant surveillance of all radioactive material and had the ability to lock it in their vehicle if needed. The inspector observed two administrations of Tc-99M for stress tests and noted that the NMT wore appropriate dosimetry and decay corrected the dose to ensure that it was within the acceptable range prior to administration. All potentially contaminated waste generated on site was stored in a receptacle and taken to the licensee's main office for decay-in-storage. The NMT possessed a calibrated survey meter to perform area surveys upon completion of administrations for the day. Licensee staff also demonstrated and discussed procedures for package receipt, area surveys, spill response, and waste handlin Through these observations and discussions, the inspector found the licensee's staff to be knowledgeable ofradiation protection principle The inspector conducted independent and confirmatory surveys of the facility and found no residual contamination or exposures to members of the public in excess ofregulatory limit No violations ofNRC requirements were identified during this inspectio NRC FORM 591M PART 3 (07-2012)
