i Auxiliary Feedwater Storage Tank - Out of Specification.

This report was initiated by Incident Report 82c473.

CONDITIONS PRIOR TO OCC'JARENCE:

Mode 2 - Rx Power 1% - Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

At 1715 hours0.0198 days <br />0.476 hours <br />0.00284 weeks <br />6.525575e-4 months <br />, November 19, 1982, while running No. 23 Auxiliary Feedwater Pump in order to verify operability, the Auxiliary Feedwater Storage Tank (AFST) dropped below the technical speci-fication minimum volume of 200,000 gallons.

Action Statement 3.7.1.3.a was entered, and AFST filling was commenced.

This occurrence constituted oporation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the occurrence was personnel error.

The operator did not open demineralized water make-up valve 2DR6 far enough to maintain the minimum volume.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.1.3 requires:

With the Auxiliary Feed Storage Tank inoperable, within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> either:

a.

Restore the AFST to operable status or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />, or b.

Demonstrate the operability of a demineralized water or a fire protection /

domestic water storage tank as a backup supply to the Auxiliary Feedwater Pumps and restore the Auxiliary Feed Storage Tank to operable status within 7 days or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

CbRRECTIVE ACTION:

The AFST volume was restored to greater than 200,000 gallons, and at 1730 hours0.02 days <br />0.481 hours <br />0.00286 weeks <br />6.58265e-4 months <br />, November 19, 1982, Action Statement 3.7.1.3.a was terminated.

The operator involved was counselled as to the importance of proper operation of the AFST and the significance of the minimum required volume.

Design Change Request 2EC-0762 has been submitted to provide a redundant AFST level indication and alarm.

Design Change Request 2EC-1252 has been subnitted to convert Valve 2DR6 from manual to manual-remote operation.
