Containment Air Lock - Inoperable.

This report was initiated by Incident Report 82-057.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 94% - Unit Load 1100 MWe DESCRIPTION OF OCCURRENCE:

On March 20, 1982, while performing Surveillance Procedure SP (O) 4. 6.1. 3 on the 100 ' elevation Containment Air Lock, it was determined that the leakage rate exceeded the limit of 0.05 La at the design pressure of 47.0 PSIG, as required by Technical Specification 3.6.1.3.b.

The Air Lock was declared inoperable, and at 1810 hours0.0209 days <br />0.503 hours <br />0.00299 weeks <br />6.88705e-4 months <br /> Action Statement 3.6.1.3.a was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the Containment Air Lock leakage was a loose chain on the inner door. handwheel mechanism, which prevented the door from sealing properly.

ANALYSIS OF OCCURRENCE :

Technical Specification 3.6.1.3.a requires:

With one containment air lock door inoperable, maintain at least the operable air lock door closed and restore the inoperable air lock door to operable status within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />, or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

The air lock outer door was maintained in a closed position.

The handwheel chain was properly adjusted and Surveillance Procedure SP(O)4.6.1.3 satisfactorily performed.

At 2115 hours0.0245 days <br />0.588 hours <br />0.0035 weeks <br />8.047575e-4 months <br />, March 20, 1982 the air lock was declared operable, ard Action Statement 3.6.1.3.a was terminated.
